1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	A prelicensing inspection visit was conducted on this date by Licensing Program Analyst (LPA) Brannon, who met with Quality Assurance Specialist, Elsa Navarrete. Before visit, LPA spoke with Elsa Navarrete, as per COVID-19 guidelines. Licensee is Community Action Partnership. Licensee is requesting the following a change in capacity. The updated request is a capacity of 40 preschool children and 12 children in the toddler option program. This program will operate year round, Monday through Friday 7:30 AM to 4:30 PM. Breakfast, lunch, snack will be provided and prepared in off site at licensee's central kitchen. There is a kitchen on site to warm up food as needed. There is a sink with hot water inside kitchen, classroom to use for food preparation. Ill children and staff will utilize the bathroom located on the first floor. Ill children will be isolated in site supervisor office. Room measurements taken and reviewed with Elsa Navarrete. There are four classrooms. Classrooms A and B will be used by preschool children. Classrooms 1 and 2 will be used by the toddler option children. The total preschool square footage is 1821 which will accommodate 40 preschool children and 12 toddlers in the toddler option program. Each toddler option classroom is equipped with a changing table within arm's reach of a sink. Each classroom has a drinking fountain for indoor drinking water. Adequate storage space available for children's belongings. Outdoor storage is available for toys and equipment. Toys and equipment are age appropriate. The preschool and toddler option children have their own outside play area. Outdoor measurements taken on this date total 8596 square feet which will accommodate the requested capacity of total capacity of 52 children. Adequate shade is available in the outdoor activity area. The applicant is using rubber-pour-in-place for cushioning under the outdoor climbing structure in the preschool play yard. There is not a climbing structure in the toddler play yard. There are a total of 4 toilets and 5 sinks/hand washing fixtures in the children's bathrooms which will accommodate the requested total capacity of 52 children. There is a drinking water fountain in each classroom. Igloo with cup dispensers and disposable cups will be utilized for outdoor drinking water. CONTINUED ON FOLLOWING PAGE

The fire clearance has been received and approved for 12 toddlers and 40 preschool children.
This facility plans to provide Incidental Medical Services – IMS. A Plan of Operation that includes IMS must be submitted to the Department. The following items must be completed prior to issuing a license by 6/21/2020:

1. Install gate to keep toddlers out of the diaper changing area.
2. Changing table does not have a changing pad.
3. Paper towel dispenser is not installed.
4. Anchor cubbies.
5. Anchor shelving units.
6. Ceiling panels in the hallway need to be repaired or replaced.
7. Found dead roaches in facility. Please provide copy of pest control invoice/contract.
