25 | On this date 07/28/2023, Licensing Program Analyst (LPA) Adrian Pizano conducted an unannounced Case Management inspection. LPA met with Director Alia Dugum, toured the facility, and took a census. The purpose of today’s inspection was to discuss an incident that was reported to Community Care Licensing (CCL) on 07/11/2023.
Director stated that on 07/07/2023, Child 1 was dancing in the play area during music time when Child 1 lost Child 1's balance and fell face first into the bookcase. During the fall, Child 1 struck Child 1’s mouth on a bookcase resulting in a cut near Child 1’s mouth. Teacher Sandra consoled the child and cleaned the would. First aid was administered and Child 1’s parent was notified. Parent sought medical attention for the child and one tooth needed to be extracted and two were cracked. Child 1 was released and returned to care on 07/11/2023. LPA discussed supervision at the time of the incident, and it was determined that appropriate supervision was in place. Staff will continue to remind children not to run inside the classroom. LPA inspected the area where the incident took place and did not observe any safety hazards or areas of concern.
LPA spoke with teacher Sandra Fuentes and Guadalupe Elyza separately. Both teachers provided the same information regarding the incident, that the Child 1 was dancing and lost Child 1's balance falling face first into the bookshelf, resulting in the injury.
This appears to be an isolated incident and staff took appropriate measures to address the child’s injury, following appropriate policies, regulations, and reporting requirements.
No deficiencies cited during today’s inspection.
