On this date, Licensing Program Analyst (LPA) Jessika Thompson conducted a Case Management inspection. LPA met with facility director, Damian Bowden. A tour of the facility was made and a census was taken.

The purpose of today's inspection was to address two individual unusual incidents that reportedly occurred at the facility on 02/07/19, regarding facility staff allegedly exhibiting aggressive behavior with children in care.

Pertaining to the first incident, it was reported by a now former staff member that Staff #1 hit Child #1 on the arm. LPA interviewed staff members listed as witnesses to this alleged occurrence. Of staff interviewed, none stated or alluded to any child in care being hit by a staff member at any time.

Pertaining to the second incident, it was reported by a now former staff person that Staff #2 trapped Child #1 within a classroom by using a table to block the child from exiting. In an interview with LPA, Staff #2 disclosed that she moved a table against a wall to block Child #1 from running out of the facility. Although Child #1 was not hurt as a result of this incident, it has been determined that this action posed a potential risk to health and safety of children in care, as excessive force was exerted by Staff #2 when pushing the table against the wall, which could have resulted in a child being hit by the table. In exit interview the licensee was advised of appeals rights and was provided with Appeals Rights.

Licensee will provide staff training regarding Title 22 Regulation 101223(a)(2). This training should include appropriate measures to be taken by staff if/when a child at the facility is attempting to elope. The licensee will submit a copy of the training agenda, outline and list of attendees, to the Fresno Community Care Licensing Department by 05/22/19. In addition, facility staff will watch Community Care
