On this date, Licensing Program Analyst (LPA) Jessika Thompson met with Director Rebecca Jarrett for an unannounced case management inspection. LPA toured the facility and a census was taken. An Unusual Incident Report was recently submitted to the Fresno Community Care Licensing Office (CCL) regarding an incident that occurred at the facility on April 25, 2019, where a day-care child slipped off of a slide in the outdoor play area, resulting in the child sustaining a fracture. The purpose of this inspection was to investigate the aforementioned occurrence. Today, LPA spoke with the attending teacher, Dawn Jones, who stated the child was running up the slide when he fell. Teacher Jones stated that she advised the child not to run up the slide, but child continued to do so, which resulted in the child falling and hurting his wrist immediately following her warning. First aid was reportedly administered, and the child’s mother was notified. The child was reportedly seen by a physician, who indicated the child sustained a hairline fracture to the right wrist. LPA observed the play area where the injury took place. The play structure and slide are age-appropriate with ample bark cushioning in the surrounding areas. Appropriate teacher-child ratio was reportedly in place when the incident transpired and attending staff stated direct supervision was being provided at the time of the incident. The child, who returned to care two business days subsequent to the incident occurring, is doing well, as per staff reports.

This appears to be an isolated incident. It has been determined that staff took appropriate measures to address the child's injury, following appropriate Agency policies and reporting procedures.

Per California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiency was cited during today's visit.

An exit interview conducted with Director Rebecca Jarrett and a copy of this report was provided and discussed. A Notice of Site Visit Form was posted on parent's board and must remain posted for 30 days.
