25 | On 8/17/2022, Licensing Program Analyst (LPA) Theresa Marquez conducted a Case Management inspection and met with Director Rebecca Jarrett, Assistant Director Dawn Jones and Teacher Alyssa Barela.
The purpose of this inspection is in regards to a self reported incident that occurred on 7/27/2022 at about 8:45AM. Two teachers were outside with 19 children when a child, under the age of 3 years was playing in the play area restricted to 3 years and over. The child was playing at the base of a dome climber (play structure). The child's foot slipped, causing the child to fall forward and hit his mouth on the metal bar of the climber, which resulted in the child's 2 front teeth being pushed into his upper gums. First aid was administered and the child's parents were called. Parents took their child to see a dentist the same day. The child was referred to a specialist and oral surgery was not needed. The child did not return to the school.
Director Jarrett stated staff will no longer permit two-year-old children to use the dome structure as is may be to advanced for their usage.
Per California Code of Regulations Title 22, Division 12 Chapter 1, the following deficiency is being cited today: All play equipment and materials used by children shall be age-appropriate. (See page LIC809-D).
An exit interview was conducted and this report was reviewed with Director Rebecca Jarrett . A copy of Appeal Rights and the Notice of Site Visit was given and must remain posted for 30 days
