1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	A prelicensing inspection visit was conducted on this date by Licensing Program Analyst (LPA) Brannon, who met with Senior Director, Alesha Hixon. The center is located at Wonderful College Prep Academy. The licensee is requesting a capacity of 80 preschool children. This program will operate year round, daily from 5:00 AM to 5:30 PM, from Monday thru Friday. Breakfast, lunch, and afternoon snack will be provided and prepared on site at the Wonderful College Prep Academy cafeteria. The classrooms have an area with an adult size sink. Hot water is turned off. Ill children and staff will utilize the bathroom located in the preschool office. Ill children will be isolated in the preschool office until the parent picks up their child. Room measurements taken and reviewed with Alesha Hixon. There are three classrooms that will be used by preschool children. The total preschool square footage is 3355 which will accommodate the requested capacity of 80 preschool children. CONTINUED ON FOLLOWING PAGE

Adequate storage space available for children's belongings. Outdoor locked storage space is available for toys and equipment. Toys and equipment are age appropriate.
Outdoor measurements taken on this date total 11898 square feet which will accommodate the requested capacity of 80 preschool children. Adequate shade is available in the outdoor activity area. Licensee is utilizing permanent shade structures. At this time, licensee is utilizing temporary cushioning around the poles until the permanent cushioning arrives. The permanent cushioning has been ordered and will arrive within four weeks. The facility has fencing around the preschool site, including the outside play yard. The school main office or preschool office personnel utilizes cameras to allow access and departure from the preschool site. Licensee utilizes barriers in two areas to keep the children on the play yard.

There are two climbing structures. One structure consists of a rope climbing structured referred to as: Multi Challenge Climber. At this time, license shall keep the children off structure until reviewed with Licensing management. The structure's highest point is 7 feet tall. Cushioning is rubber-pour-a-form.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. During today's visit, LPA provided a handout consisting of: Provider Information Notices; Child Abuse Mandated Reporter Training that meets AB1207 requirements; Effects of Lead Exposure and Safe Sleep.

Pending a final file review, a recommendation will be made to license the above facility for a capacity of 80 preschool children. Licensee shall send in an updated application with capacity of 80 preschool children, board resolution approving capacity of 80 preschool children, a check for the funds of $484.00, and a fire clearance for 80 preschool children.

The second climbing structure has a 270 degree slide. Above the three slides, there are bars that the children can grab and swing out. LPA is recommending that staff remind children to sit down and slide. There is a tunnel feature with the climbing structure. LPA was informed that only one child at a time can go through the tunnel. This will assist with the required visual supervision of children. For this area, licensee is utilizing wood chip as cushioning.

There are six toilets and six sinks/hand washing fixtures in the children's bathrooms which will accommodate the requested preschool children. There are water fountains in the outside play area to be utilized as drinking water while the children are outside. Each classroom utilizes water dispensers, cup dispensers and disposable cups for inside drinking water. Each classroom has one carbon monoxide device.

The fire clearance has been received and approved for 60 preschool children. Licensee has enough square footage for 80 preschool children. Upon returning to the office, LPA will request an updated fire clearance for a capacity of 80 preschool children.
