On this date, Licensing Program Analyst (LPA) Gloria Reyes met with Site Supervisor, Hilda Osorio for an unannounced Case Management - Incident inspection. LPA toured the facility and a census was taken. An Unusual Incident Report was submitted to the Fresno Community Care Licensing Office regarding an incident that occurred on 10/16/19, at approximately 10:51 AM in Room 104, where a day care child exited the back door of the classroom that lends to the play yard.

On the day of the incident, LPA verified that there was 3 staff and 15 children present in the classroom. Child #1, exited the back door at 10:51 AM. Staff #1, was standing in the immediate area where child #1 was at. However, staff #1 had his/her back toward child #1, and he/she did not observe the child exit the classroom. The TK teacher who was outside with her students observed child #1, at the water fountain and assisted the child at the fountain. A minute later, the TK teacher returned child #1 to his/her classroom. A security camera also verifies that the information that was reported on the Unusual Incident Report/Injury Report (LIC 624), as accurate.

Today, LPA observed the area where the incident took place and LPA did not observe any hazards. The play yard is completely enclosed by a fence. LPA inspected the door and verified that it is properly functional and closes. The facility also had the Maintenance Department inspect the door to ensure that it continues to close. The Site Supervisor has reminded her staff to physically pull door behind them and visually see that the door has closed. Site Supervisor informed LPA that a door chime will be placed at each door as an additional measure of security to ensure the safety of children at the facility.
This appears to be an isolated incident and staff took appropriate measures to address the incident and followed appropriate Agency policies and procedures.

Per California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiency cited today.

An exit interview was conducted with Site Supervisor, Hilda Osorio and a copy of this report was provided and discussed. A Notice of Site Visit Form was posted on parent's board and must remain posted for 30 days.
