25 | On this date, Licensing Program Analyst (LPA) Gloria Reyes met with Site Supervisor, Hilda Osorio for an unannounced Case Management - Incident inspection. LPA toured the facility and a census was taken. An Unusual Incident Report was submitted to the Fresno Community Care Licensing Office regarding an incident that occurred on 12/11/19, at approximately 9:24 AM. A day-care child who was jumping in front of the door of classroom 104, tripped, hit the lower hinge of the door hitting the right-side temple area and, sustained an open wound. 911 was called. While the ambulance was in route, the Family Nurse Practitioner (FNP) from the on-site Wellness Center came to assist the child. The FNP and the Lead Teacher applied pressure to the injury. When the ambulance arrived, the bleeding had stop. The parent was contacted after 911 was called. The parent arrived prior to ambulance arrival and parent accompanied the child via ambulance to the hospital. The child was transferred to Delano Regional Medical Center. The child was treated, received two stiches and was released. The child returned to school today (12/18/19) and is doing fine.
The Site Supervisor had a meeting with her staff on 12/12/19. Staff watched a CCL Video related to Care and Supervision. Staff are to remind children to be more observant of their surroundings. Staff also reviewed Zone Positioning when outside with children. Picnic tables near the classroom door were removed. Site Supervisor Hilda is also purchasing whistles for staff to utilized when outside with children. LPA was provided a copy of the agenda and sign-in sheet as well as other documents.
LPA observed the area where the incident took place. LPA noted that the exit door to classroom 105 does not have a "painted" line to show that the door opens away from the classroom. LPA recommended to Ms. Hilda that it would be a best practice to have a safety line painted in front of all doors that exit to the outside play yard. LPA recommended to staff that children are to be reminded not to play near the classroom doors. Also, LPA verified that teacher-child ratios were in compliance. (see next page) |
