NARRATIVE
1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On this date, 3/17/20, an announced prelicensing inspection visit was conducted by Licensing Program Analyst (LPA) Brannon, who met with Quality Assurance Specialist, Elsa Navarrete. The licensee is requesting the following a total capacity of 23 children: 15 preschool children and 8 toddler children. This program will operate year round, Monday through Friday from 7:30 AM to 4:30. Breakfast, lunch and PM snack will be provided and prepared off site in applicant's central kitchen. There is no kitchen on site. Ill children and staff will utilize the bathroom located in the hallway. Ill children will be isolated in director's office. Room measurements taken and reviewed with Elsa Navarrete. There are two classrooms. One is for the preschool children and the other is for the toddler children. The total preschool square footage is 1123 which will accommodate 15 preschool children and 8 toddler children. Adequate storage space available for children's belongings. Outdoor storage is available for toys and equipment. Toys and equipment are age appropriate. Outdoor measurements taken on this date total 4621 square feet which will accommodate the requested capacity. Adequate shade is available in the outdoor activity area. Applicant installed two permanent shade structures with cushioning wrapped around the metal shade poles. There are 3 toilets and 3 sinks/hand washing fixtures in the children's bathrooms which will accommodate the requested capacity of 23 children. There are drinking water fountains, one in each room. An Igloo will be utilized for outdoor drinking water with disposable cup dispenser. CONTINUED ON FOLLOWING PAGE

The fire clearance has been received and approved for 23 children.

This facility plans to provide Incidental Medical Services – IMS. A Plan of Operation that includes IMS must be submitted to the Department. The following items must be completed prior to issuing a license by 3/20/20. Applicant will email the pictures:
1. Anchor cubbies and shelving units that can be tipped over.
2. Replace the broken grate on the exterior wall.
3. Remove the carpets in the outside play yard.

Pending a final file review and completion of above items, a recommendation will be made to license the above facility for a capacity of 23 children: 15 preschool children and 8 toddler children. Please update LIC 500, LIC 610, LIC 308, Facility Sketch upon licensure.
