On 04/13/2023, Licensing Program Analysts (LPAs), Jose Penate and Martha De Haro, conducted an unannounced Annual Required Inspection and were met by Licensee’s Assistant Laila Haulcy. Also present was licensee’s niece. Days and hours of operation are Monday – Friday, 7am to 6 pm.

LPA toured the home inside and outside and a census was taken. Current facility sketch reviewed, and Licensee confirmed rooms accessible for children to use is the daycare room, living room, bathroom, dining room, kitchen, and backyard. During outdoor inspection, LPAs observed gardening tools and a plastic gas can containing gas accessible to children in care. Licensee immediately removed gas can from the area and placed it in a locked shed. There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition on the premises. During inspection, LPAs observed hand sanitizer and medication that was accessible to the children in the living room as well as clutter (high piled cooking items-high stored toys-etc). In the kitchen, LPAs observed kitchen knives on the kitchen counter as well as a can of paint on the floor. LPAs also observed clutter on the kitchen counter. In addition, LPAs observed cleaning products, tools, and a paint can in the laundry room, that are accessible to children in care.



There is a working fire extinguisher, smoke detector, carbon monoxide detector and adequate heating and ventilation for safety and comfort. The home has working telephone service and LPA confirmed the phone number is (661) 302-1394.

LPA and licensee discussed Safe Sleep Regulations. LPAs observed an empty bottle in one of the cribs with no child present. LPAs discussed regulations with Licensee and made her aware that cribs and play yards are to be kept free from all loose articles and objects while infants are sleeping, and there are no objects hanging above or attached to the crib or play yard.

Provider is aware to physically check on sleeping infants every fifteen minutes but she was unaware that she needed to document any signs of distress which includes but is not limited to flushed skin color, increase in body temperature, restlessness and labored breathing. Individual Infant Sleeping Plan is to be completed for each infant up to 12 months of age upon enrollment. LPA discussed with licensee, Infants up to 12 months of age shall be placed on their backs for sleeping. While discussing safe sleep regulations, licensee confirmed that she has not completed LIC 9227 forms for infants in care that are 12 months of age or younger.

Licensee ensures that children in care are supervised at all times and is aware children shall not be left in parked vehicles. Car seats are used for transportation purposes only and are not used for sleeping children. During inspection, LPAs observed 3 car seats accessible to the children in the living room and kitchen. LPAs informed licensee that car seats should not be accessible to children and licensee removed them from the area immediately. Capacity as specified on the license is being maintained.

LPA reviewed a sample of children’s files and observed files were complete with emergency information as required. Licensees Mandated Reporter Training was completed on 7/17/2020, licensee stated she recently completed the training with the union but is unable to gather her completed certificate. Licensee’s pediatric CPR/First Aid expires on 02/26/2025. A review of records indicates that Assistant was unable to provide immunization records (MMR, Tdap, Influenza, and TB). All adults who reside or work in the home do not have a criminal record clearance or exemption. During inspection, LPAs observed an uncleared adult present at the facility, assisting with children in care.

Incidental Medical Services (IMS) are not currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, deficiencies are being cited on the attached LIC 809-D.
