On 52/27/2021, Licensing Program Analyst (LPA) Ruby Ocegueda conducted a case management inspection for the purposes of addressing some observations made during a complaint inspection on 4/27/2021. LPA provided identification and a Covid-19 screening was conducted upon arriving today. A tour and census was taken.

On 4/27/2021, while taking a census and taking a tour of the facility, LPA Ocegueda and LPA Araceli Gibson observed the following: There was one child in care and there was one staff present (Staff #1) at the time of arrival. LPA's observed that the half metal gate leading to the kitchen was unlocked and open (photo taken). The kitchen had accessible cleaning supplies ("Pine and Go", "Super Strength" Bleach and other unmarked liquid solutions, approximately 12 in all). The cleaning supplies were in the cabinet under the sink which had a child safety mechanism that was not latched. A round pizza cutter was also observed in an accessible drawer next to the sink. The observations in the kitchen were immediately discussed with staff #1 and she removed them and made the cleaning supplies inaccessible that day. Also a closet that was described as a utility closet that was located near the kitchen and child restroom had paint and a unknown sharp gardening tool. The closet was accessible. Photos and video were taken. This observation was also immediately discussed with staff #1 and she stated she would make the closet inaccessible.

On 4/27/2021, while conducting a complaint inspection, LPA's Ocegueda and Gibson observed that staff #1 was caring for 18 children outside in the play yard and that two children were out of staff#1's line of vision causing staff #1 to not provide the required direct supervision (lack of supervision, cited on complaint inspection report, see 9099 and 9099 D dated 5/27/2021). LPA's observed for several minutes, Staff #1 did not count the children or ask for other staff to assist her to bring her back to ratio. Children were observed to have free range of the facility during the inspection on 4/27/2021 (walking between the main building, the play yard and the bungalow building). Report continued on 809-C

On 4/27/2021 and 5/27/2021, LPA discussed these observations with licensee and reminded licensee that although children had free range of the facility, licensee still had to ensure that required staff to child ratios were being followed at all times, whether Licensing was present or not. Licensee stated she would be working to ensure she would follow ratio as she was still training new staff.

On 4/27/2021, LPA's also observed a uneven area of dirt that is located between the main facility building and the bungalow. The area of dirt was hard and uneven. LPA discussed this with owner Carrie and informed her that this was concerning because it could cause tripping while the children used the area. There was also plastic play equipment that was located in the same area that was very dusty and appeared had not been cleaned recently. Licensee confirmed that the dirt area was uneven and had a large whole in it. Photos and video taken.

These observations were discussed with staff #1 and licensee on 4/27/2021 in order to bring to licensees immediate attention and explained that LPA would be re-addressing them further with a formal case management report. Licensee stated she understood.

Today 5/27/2021, LPA revisited observations made on 4/27/2021 (as noted above).
LPA also reviewed Covid-19 precautions that are recommended by the Department and provided a copy of the Covid-19 Self Assessment and postings to assist licensee in staying consistent with Department guidance. LPA reminded licensee to follow Covid-19 precautions as best the facility could and to continue to encourage that parents report any possible exposures or any Positive tests so as to help reduce the risk of spread. LPA requested that licensee please contact LPA or the Department to report any Unusual Incidents related to or unrelated to Covid as well as reviewed timeline for reporting to the Department.

LPA encouraged licensee to communicate any questions she had about her facility to LPA and encouraged licensee to review Provider Information Notices (PINS), the agencies website (ccld.ca.gov) to review further information and guidance.

Teacher-Child Ratio. A teacher shall supervise no more than 14 children or with an aide a maximum of 28 children. This requirement was not met as evidenced by: Observation and interview. On 4/27/2021, LPA's Ocegueda and Gibson counted 18 children under the supervision of staff #1 while in the play yard.

LPA's reviewed this observation with licensee, and she stated that she had new staff that she was still training. This poses an immediate risk to the Health, Safety and/or Personal Rights of children in care.
