On 03/20/2025, an unannounced Case Management - Plan of Correction (POC) Inspection was conducted by Licensing Program Analyst (LPA) Lady Cabrera. LPA met with Staff Ana Solorio and spoke with Evelyn Solorio, Program Administrator of Operations over Microsoft Teams meeting to review the POCs associated to deficiencies cited on 02/24/2025.

Upon arrival, staff reported Director Yasmin Juarez was not at the facility. LPA requested to see the staff files. Staff did not have access to electronic files. LPA did not have access to review electronic files again. Deficiency has not been cleared. Program Administrator reported she will update the written plan for all personnel records to be readily available for review and will submit by Friday, 3/21/2025.
