NARRATIVE
1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	A prelicensing inspection visit was conducted on this date by Licensing Program Analyst (LPA) Cynthia Brannon, who met with licensee, Veronica Puga. Licensee is requesting an increase in capacity. This program will operate year round, from 7:00 to 6:00, AM/PM from Monday thru Friday. Breakfast, lunch, and two snacks will be provided and prepared at facility. There is a sink with hot water inside kitchen, classroom to use for food preparation. Ill children and staff will utilize the bathroom located in the classroom. Ill children will be isolated in library part of the classroom. Per licensee, one staff will be assigned to provide supervision to the sick child, until the child is pick up by parent. Room measurements taken and reviewed with licensee, Veronica Puga. There is one classroom that will be used by preschool children. The total preschool square footage is 1217, which will accommodate 34 preschool children. Licensee is utilizing a water dispenser with disposable cup dispenser and cups. Adequate storage space available for children's belongings. Outdoor storage is available for toys and equipment. Toys and equipment are age appropriate. Outdoor measurements taken on this date total 6548 square feet which will accommodate a capacity of 34. The outside play area is blacktop, with no trees. Per licensee, a shade structure will be installed this weekend. Licensee is utilizing a canopy shade structure for shade. There is not an outside drinking water fountain, a pitcher will be utilized for outdoor drinking water with disposable cup dispenser and cups. CONTINUED ON FOLLOWING PAGE

There is no climbing structure on site during today's visit.
There are 3 toilets and 3 sinks/hand washing fixtures in the children's bathrooms which will accommodate 34 children.
Per Veronica Puga, at this time, she is not providing Incidental Medical Services (IMS). Per Veronica Puga, she is aware and does have IMS in place. There are no medication on site during today's visit.
The fire clearance has been received.
The following items must be completed prior to issuing a license by:
1. A shade structure shall be installed as stated by licensee. Cushioning around the poles shall be installed to provide cushioning when a child runs into the poles. This can help in preventing a potential serious injury to the child. Licensee will notify LPA for a return visit to inspect the shade structure.
Pending a final file review and completion of above items, a recommendation will be made to increase capacity to 34 preschool children.
