25 | Prior to today's inspection, LPA telephoned call applicant and conducted the COVID-19 Emergency Response Tele-Inspection Screening Process. On May 12, 2022, Licensing Program Analyst (LPA), Norma Lomeli met with Applicant, Chantell Warnock for a pre-licensing/change of location inspection. Applicant, her Fiance, and five minor children reside in the home. Verified applicant’s CPR and First Aid was completed through American Red Cross and expires on July 24, 2023. Applicant’s assistant, Bennett Stewart completed the training through American Red Cross and expires on April 17, 2023. Background clearances are discussed and LIS 531 is signed indicating that the adults currently living in the home and/or providing care and supervision to children have a criminal record clearance or exemption. Fire clearance was granted on May 9, 2022.
Facility was inspected inside and outside as shown on the facility sketch and the following items were discussed:
- Fire clearance was received on May 11, 2022. Fire alarm is located on the home’s hallway wall.
- This is a two story, four bedrooms and three bathrooms home and upstairs area will be off-limits to the day-care children. There is a gate at the bottom of the stairs making upstairs area inaccessible. Care and supervision will be provided in the living room, family room, dining room, kitchen, downstairs bedroom (day care room) and downstairs bathroom.
- There is an electrical fireplace in the family room. Applicant states that she does no use fireplace during day care hours.
