The facility pre-licensing inspection was conducted by Licensing Program Analyst (LPA), Wisehart. The applicant has applied for a capacity of 45 preschool age children under a change of ownership. The facility was toured inside and outside. The center will operate from 6:30 am- 5:30 pm, M-F. The preschool facility consists of 2 classroom(s) serving ages 2 to 5 years old. The Classrooms are in a building on the campus of Meadowview Elementary school. Classroom #1 and #2 will be used for preschool ages 2-4 and they have an office between the rooms. The office has windows to each classroom and pocket doors to close off the office if needed. Room 1 & 2 combined measure for 52 children and the play yard measured for 126 children. The fire clearance was approved on 11/10/21 for 45 preschoolers. Therefore, the requested capacity is approved for 45 children. Required postings (Parents Personal Rights, Emergency Disaster Plan, Earthquake Preparedness Checklist, Menu, etc.) listed on Form LIC 311A are posted by the entrance. The applicant was advised that all staff are required to have a criminal background clearance on file with Community Care Licensing. The applicant is aware of the immediate $100 per day civil penalty for anyone providing care or supervision without a criminal record clearance.
The Director meets Title 22 requirements. Children will use the fenced outdoor play area. Sign-In/Out procedure were reviewed. The applicant/director understands that if a manual Sign-In/Out procedure is used; full legal signatures are required. Cubbies are available for children to use. There are pull alarms, and there is a working carbon monoxide detector and charged fire extinguisher(s) in the facility rated at least the 2A10BC and each room is tied into the bell alarm with the elementary school. Three bathrooms will be used for room 1 & 2 and each room has 2 sinks and faucets. There are 3 toilets for children. There is one staff bathroom, which is separate on school grounds. There is an indoor drinking fountain available and outdoor drinking fountains, and children are also provided water cups. The isolation area for a sick child will be established in the office and a mat will be provided. The facility has sufficient cots available for children and bedding will be washed on site weekly. Parents are responsible to provide breakfast and lunches and the program provides a morning and afternoon snack.

The food is properly stored, and refrigeration is provided. Age appropriate toys and activities are available for children such as riding toys, balls, water tables, and activities for children. Children will have access to a fenced outside play yard which has a large locked wooden storage shed, blacktop, grassy area and a play structure on wood chips sufficient to absorb falls. There are trees present to provide shade in the play yard. There are no bodies of water located on the property and none are to be added without prior notification and approval of the licensing agency. Incidental Medical Services (IMS) policy was discussed. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. This report, as well as the Guide to Safe Sleep Practices brochure, Lead Exposure Testing and COVID Guidance were reviewed and discussed with the applicant/director. All licensing reports are public information and must be made available upon request for at least three years.
