On 11/18/2024 at 11:45 AM, Silva Garibyan, Licensing Program Analyst (LPA), conducted an unannounced Annual Random Inspection for the preschool license. LPA met with Shelby Fine, Assistant Director and toured the facility indoors and outdoors. Days and hours of operation are Monday through Friday from 7:30 AM to 6:00 PM. The facility is located on the same premises that Citizens of the World East Campus Charter Elementary School is located in. There are two armed security guard at the entrance of the parking lot. The Preschool Program occupies 9 classrooms (Rooms 1-9) and two playground. The facility is currently using classrooms 1, 2,4,5 (Art Room), 6,8,9. Rooms 1-6 are located in front of the facility office and there is a playground in front of this classrooms. Rooms 7- 9 are located next to the backside of the office and have a separate playground. There are working telephones, first aid kits, and smoke/carbon monoxide detectors in each classroom. There are pull down fire alarms and fire extinguishers (serviced 01/15/2024) located in the breeze ways of the facility. Upon arriving at the center, LPA observed 58 children being supervised by 12 teachers. LPA observed two children in Room 2 (2 year olds) supervised by two teachers, 14 children in Room 4 (3 year olds) supervised by three teachers, 13 children in Room 6 (Pre-K, 4 year olds) supervised by two teachers, 15 children in Room 8 (3 year olds) supervised by three teachers, 14 children in Room 9 (3-4 year olds) supervised by two teachers. Facility is operating within capacity limitations. LPA verified that all adults present in the facility have obtained criminal record clearances and are associated to the facility. The facility is currently utilizing the electronic sign in/out (Bright Wheel). Disaster drill log was available during today’s inspection, last disaster drill was conducted on October 22, 2024. Per Assistant Director, isolation area for sick children is in the office. There is no swimming pool or other bodies of water on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked storage area. Page 1

Fixtures, furniture and equipment are adequate and well maintained. LPA observed the following postings by the office: License, LIC 613A, the PUB 393, Child Care Center Notification of Parents' Rights Poster, LIC 610 Emergency Disaster Plan with the telephone number of the local health department, information on child passenger restraint systems (PUB269). Furniture and equipment were inspected for age appropriateness and good repair. The classrooms were inspected to ensure that the floors have a surface that is safe and clean.

A walk through of the classrooms was conducted, classrooms were found to be clean and free from any potential hazards. Furniture was found to be in good repair and age appropriate.

There is adequate heating, lighting and ventilation. Drinking water is readily available in each classroom.Drinking water is available both indoors and outdoors via personal water bottles. There are first aid supplies in each classroom and in the office. .

Napping equipment and bedding was inspected for good condition, appropriate storage and cleanliness. LPA observed mats available for each child are being sanitized once a week. Bedding is stored separately. Napping children are properly supervised within ratios. Isolation area for sick students is located in the office. LPA observed adequate arrangements for isolation and care of ill children.


LPA inspected and observed a total of 5 clean bathrooms. There are a total of 14 toilets and 12 sinks available for children's use. Toilets and sinks are functioning properly and are age appropriate. LPA observed soap, toilet paper and paper towels readily available. Water temperature was tested and is appropriate Water temperature was tested and is appropriate. Bathrooms were found to be clean. There is adequate lighting/ventilation in the bathroom areas.
Children are inspected for illnesses as they arrive. A review of medication policy indicated that Medication is administered by Director, who documents the date, time and dosage onto a log. Medications are stored securely and inaccessible to children in the office. Page 2

There are 2 kitchens on the premises. One kitchen is located near the Preschool and is equipped with refrigerator, freezer and microwave oven. Snacks are stored in air tight containers. The facility provides two snacks daily (three snacks for extended care) and offers a hot lunch upon request; other children bring their lunch. Hot lunches are prepared in a separate kitchen. Menus are posted in each classroom. LPA observed an appropriate amount of food and snacks. The chemicals are kept separate from the food and there is hot running water in both kitchens. The preschool kitchen maintains a list of children with allergies and a copy is also kept in the child's classroom.
Outdoor equipment was inspected for safety, cushioning material, good repair and age appropriateness. Required shade, drinking water and fencing were inspected. Fencing around the perimeter of the play area is at least 4 feet high. Play area was inspected for hazards and inaccessibility to bodies of water. The facility is free of flies, insects and rodents.

LPA conducted a Children's Records Review for completeness and to ensure children have proper forms such as Identification and Emergency forms, Immunization records, Health History, Personal Rights, Parents Rights, Sleep Plan (LIC 9227) and sleep lot on file. The children's roster was available and current. Staff Records were reviewed to ensure the following: Criminal record clearance or criminal record exemption, education credits required for Infant Care, current CPR/First Aid and health preventative practices, proof of immunization against pertussis and measles and influenza and/or flu declination, Mandated Reporter training. LPA observed at least one staff member to have current EMSA approved pediatric CPR/1st Aid.

Assistant Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

A notice of site visit was given and must remain posted for 30 days.
