On 12/21/2022 at 07:55am, Licensing Program Analyst (LPA) Denise Miranda, conducted an unannounced Required - 1 year Inspection for the preschool License. LPA met with Michele Vault Jimenez, Director and toured the facility indoors and outdoors. Days and hours of operation are Monday-Friday 7:00 AM – 6:00 PM. LPA, observed 27 children in care and 5 staff providing care and supervision. Facility checklist (LIC125) provided upon arrival to Director.

The facility has toddler option at the preschool program. LPA observed designated cubbies in each classroom. LPA observed age appropriate equipment in each classroom and observed furniture and equipment in good condition.

Per License certificate the classroom#9 has been designated to the toddler component. LPA observed facility using classroom #1 #2 for toddler component and classroom#9 for preschool program. Director will move back the toddler option to the classroom #9 and the preschool children that were using the classroom #9 will be move to the classroom #5 and #7. There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked storage area. No poisons were observed during the inspection.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space needs to be maintained clean, no hazards was observed. Outdoor activity area is fenced all around and is supplied with age and size appropriate equipment, label of the age appropriate will be place. An adequate amount of cushioning material turf is in place under play. page 1 of 6.

The restroom is located outside the classrooms, it was inspected and was noted to be clean and equipped with good flushing toilets, soap dispenser, paper towels. LPA observed one sink at boys bathroom that was place as off limit and needs repair.
Adequate shade is provided in outside play area. Drinking water is provided in the outdoor play area children water bottles. Facility has extra bottle of water available for the children in care.

Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption.

Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities.

The name of the preschool child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department.

The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 preschooler per full qualify teacher and one teacher supervising no more than 6 toddler per full qualify teacher. LPA reviewed a sample of 5 children’s files and observed files were no completed with child and medical assessment. LPA reviewed all staff files present during this inspection and observed files were not complete due a missing immunization. Menus are posted at least one week in advance where an authorized representative can view them, however, facility missing to provide the two weeks menu calendar.

Criminal Record Statement :Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA and Director discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

New Appeal Process: A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment. All appeals must be sent to: California Department of Social Services | Community Care Licensing Division | 300 N. Continental Blvd. Suite 290-A |El Segundo, CA 90245

Assembly Bill 1207: California Child Care Workers; Mandated Training Requirement. Beginning January 1, 2018, all licensed providers, applicants, directors and employees must complete Mandated Reported Training prior to March 30, 2018 and renew training every two years at: www.mandatedreporterca.com.

Nutrition Requirement: Beginning January 1, 2016, AB 290 will require for each new license issued, at least one director or teacher at each child care center or family child care home to have at least one hour of training in the importance of childhood nutrition. This applies to anyone submitting a new application, relocating their facility, selling their facility or transferring their license. Please note this training cannot be completed online or by home study programs. The training must be taken from an Emergency Medical Services Authority (EMSA) approved training program OR an accredited college or university. www.emsa.ca.gov.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018 requires all licensed Child Care Centers (CCC’s) constructed before January 1,2010 to test their drinking water for lead contamination between January 1, 2020 and January 1, 2023, and then every five years after the date of the first test.

LPA provided to the Director a copy of the Provider Information Notice (PIN) 21-21-CCP and Lead Testing in Child Care Centers Frequently Asked Questions.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
