Licensing Program Analyst (LPA) Dayna Chambers conducted an unannounced required 3-year annual inspection to the above facility on April 18, 2024. LPA arrived at the facility at 12:30PM, identified self and met with Miriam Mancilla, Director, who guided analyst on a tour of the facility. LPA provided Licensee/facility representative with a copy of the LIC 125 Entrance Checklist to help facilitate the inspection. This is a preschool program which consists of 6 classrooms. The center is located inside the First Christian Church. There is an unarmed guard on the property 24 hours a day. Upon arrival, the following staff were present during this inspection: RM. 1-Grasshoppers (ages: 2) with 12 children and 2 staff, RM 2-Ladybugs (ages: 2.5) with 9 children and 2 staff, RM 3-Hummingbirds (ages 3) with 10 children and 2 staff, RM 4-Bumble Bees (ages 3.5) with 9 children and 2 staff, RM. 5- Sunflowers (ages 4 ) (ages with 10 children and 2 staff, and RM. 6- Dragonfly (age: 4.5) with 7 children and 2 staff. Facility operation hours are Monday through Friday from 8:00 AM to 5:00 PM. All areas identified on the Facility Sketch were inspected. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility: Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are taken home each week to be washed. Napping equipment (mats) were observed in a separate storage room. Per director, the isolation area is located in the office. Age-appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of all drinking water is provided by the parents for their children. There is also a Brita filtered water dispenser and cups. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Director states that poisons are locked in the custodian cabinet which is located outside front of building in a locked room. Carbon monoxide detectors were observed and are operable. The center does not use the kitchen for children. The parents provide all of the children’s snacks and meals. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each meal.

Outdoor playground equipment is in a safe condition, free of sharp, lose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed from children’s own containers. All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free There are no firearms stored on the premises. There are no pools or bodies of water at the facility. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. Children’s Records were reviewed. The name, address, and telephone number of child’s authorized representative is on file. Inspection of required forms was made and documented on the LIC 857. LPA also reviewed staff records. The review of Staff records was documented on the LIC 859. Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file. Staff present did have proof against MMR, TDAP, Influenza or declination, and TB Clearances. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions.Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed via a report from Bright wheel. Children present were signed in. Disaster drill log was available, last drill was conducted on 02/08/2024. The center does not provide meals and snacks. The meals and snacks are provided by the children’s individual parents. First Aid supplies were observed in the classroom in a cabinet. According to Director, medication is only administered to a child when accompanied with a doctor's note and is stored in the office. The Director will update their Incidental Medical Services Statement. To improve the quality and value of the new inspection process, a survey may be sent to the email address provided. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).LPA verified that the lead testing was completed in accordance to the Written Directives outlined

LPA also informed Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.



This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Director will update the IMS statement.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

A notice of site visit was given and must remain posted for 30 days.
Exit interview conducted and report was reviewed with the Miriam Mancilla, Director.

There were no deficiencies cited during today’s inspection.
