On 11/09/2022, 12:55PM, Licensing Program Analyst (LPA) Denise Miranda, conducted an unannounced Annual Required Inspection for the preschool License. LPA met with Allyne Baker, Director. Days and hours of operation are Monday-Friday 8:00 AM – 3:00 PM. LPA, observed 14 children in care and 4 staff. Facility checklist (LIC125) provided upon arrival to Director.

There are two classrooms designated to the preschool program. Facility is located on the Congregational Church of Chime Nursery School Campus.

There is a swimming pool and fountain by the church campus. Per Director, Children do not have access to the swimming pool or the fountain area. LPA observed that swimming pool that has fence a fence higher than 5 feet, with a gate and lock. No children were observed on this area during this inspection. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked storage area. No poisons were observed during the inspection.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Outdoor activity area is fenced all around and is supplied with age and size appropriate equipment. An adequate amount of cushioning material (sand) is in place under play.

Facility has one bathroom located inside of the classroom wonder kids (with one sink and one toilet) and two additional bathrooms located at the hallway of the facility, that LPA

Based on interview with licensee and record review, the licensee did not comply with the section cited above in Director and staff #1 and #3 had a mandated certificate expired and staff#2 and #4 they do not have mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

inspected and observed the following: girl’s bathroom (3 sinks and 3 toilets) and the boy’s bathroom (3 sinks, 2 toilets and 3 urinals). It was inspected and was noted to be clean and equipped with good flushing toilets, soap dispenser, paper towels.
Drinking water is provided in the outdoor play area via filter water and all children have their own water bottle. Per Director, facility provided the morning and afternoon snacks and parents send their own lunch. LPA observed that facility has available food available. LPA observed, sink, refrigerator, and microwave in each classroom. This area are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair.
Facility has functioning carbon monoxide detectors in each classroom that meet statutory requirements and a working fire extinguishers.
Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption.
Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at off-site activities.
The name of the preschool child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 preschooler per full qualify teacher. LPA reviewed a sample of 5 children’s files and observed files were completed with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed all staff files present during this inspection and observed files were not complete due a missing mandated reporter. Menus are posted at least one week in advance where an authorized representative can view them, however, facility missing to provide the amount and portions on their menus.

transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Incidental Medical Services (IMS) policy was discussed. Per director no currently children needs medical services.
Regulation and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm.

LPA and Director discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

New Appeal Process: A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment. All appeals must be sent to: California Department of Social Services | Community Care Licensing Division | 300 N. Continental Blvd. Suite 290-A |El Segundo, CA 90245

Assembly Bill 1207: California Child Care Workers; Mandated Training Requirement. Beginning January 1, 2018, all licensed providers, applicants, directors and employees must complete Mandated Reported Training prior to March 30, 2018 and renew training every two years at: www.mandatedreporterca.com.

Nutrition Requirement: Beginning January 1, 2016, AB 290 will require for each new license issued, at least one director or teacher at each child care center or family child care home to have at least one hour of training in the importance of childhood nutrition. This applies to anyone submitting a new application, relocating their facility, selling their facility or transferring their license. Please note this training cannot be completed online or by home study programs. The training must be taken from an Emergency Medical Services

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018 requires all licensed Child Care Centers (CCC’s) constructed before January 1,2010 to test their drinking water for lead contamination between January 1, 2020 and January 1, 2023, and then every five years after the date of the first test.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
