On 12/02/2022 @ 1:40 PM, LPA Cohen conducted an unannounced visit for the purpose of delivering the finding against alleged complaint reported concerning the above preschool. Upon arrival, LPA Cohen observed 12 adults providing care for 56 children. LPA Cohen met with the preschool director, Lesley Diaz.

After conducting interviews with two parents of children currently enrolled and three staff members (written declarations obtained), the following conclusion has been reached: Unsubstantiated - A finding that the complaint is unsubstantiated means that although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violations occurred.
An exit interview was conducted with the above items discussed with preschool director.
A copy of this report was provided.

On 9/19/2022 at 11:25 AM, Licensing Program Analyst (LPA) Dalicia Adkins conducted a 10-day complaint visit regarding allegation listed above. LPA was greeted by Director Lesley Diaz, LPA informed director about the purpose of the visit and was granted entry into the facility. Director guided LPA on a tour of the facility. There were 14 teachers and 72 children present.
LPA interviewed director and teachers. LPA collected the following supportive documents/records: parent handbook, children’s roster, teacher’s roster and diaper changing charts.
Based on interviews, observations and records reviews it was determined that allegation listed above needs further investigation. No deficiencies cited during today visit 9/19/2022. Exit interview conducted and a copy of this report provided.
