Licensing Program Analysts (LPA) Anomeh Eivazian and Katrina Chicote conducted an unannounced random inspection. LPAs met with Arsine Aghazarian, Director who guided analysts on tour of facility. This is a preschool program which consists of 10 classrooms, Pink 1 and Pink 2 (2.5- 3 years old), Purple 1, Purple 2, and Purple 3 (3-4 years old), Blue 1, Blue 2, and Blue 3 (4-5 years old), Kindergarten 1 and Kindergarten 2, Music Room, and Auditorium/Lunch Room. Facility operation hours are from Monday to Friday from 7:30 AM to 5:30 PM.

All areas identified on the Facility Sketch were inspected. The following staff was present during this inspection: RM Pink 1: 16 preschoolers with 3 staff, RM Pink 2: 12 preschoolers with 3 staff, RM Purple 1:17 preschoolers with 3 staff, RM Purple 2: 18 preschoolers with 3 staff, RM Purple 3: 19 preschoolers with 3 staff, RM Blue 1: 13 preschoolers with 3 staff, RM Blue 2: 19 preschoolers with 2 staff, RM Blue 3: 21 preschoolers with 3 staff, Kindergarten 1: 25 children with 2 staff, and Kindergarten 2: 21 children with 2 staff. The following was observed during tour of facility:

PHYSICAL PLANT


Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Assistant Director states that there is no poison on the premises. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. All toilets and hand washing sinks are safe, sanitary and are operating properly. All floors are clean and safe.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish and rodents and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. There is also a full kitchen with a full size refrigerator, stove,, sink with hot and cold running water; the kitchen is located next to the Auditorium.

Trash cans used to discard food have tight fitting lids. Drinking water is readily available both indoors and outdoors. The facility was observed to be free of flies, other insects and rodents.

Napping equipment and bedding was inspected for good condition, appropriate storage and cleanliness. Storage for children's belongings was inspected. Parents take linens home on Friday and returned on Monday. The Director's office will be used by ill children. There is coach available for an ill child. The staff restroom will used by ill children.

Teacher child ratios were observed and staff names recorded. Children are signed in upon arrival to each child's class. Sign in and out sheets were reviewed.

The outdoor area has been divided into several yards. Outdoor playgrounds equipment are in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity spaces are maintained in a safe condition and are free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. There are no bodies of water on the premises. There are four sand boxes on the playgrounds, that are covered every night, and before children go to the play, sand box will be checked.

FACILITY RECORDS
All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. The name of the child care center director or fully qualified teacher(s) designated to act in the director's absence is on file. Educational background, training, mandated reporter training, and/or experience, health screening for each staff present are on file and were reviewed.

In review of children’s records, files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.

Children's roster was reviewed and is current. Disaster drill log was available, last drill was conducted on 11/12/19.

Snack menus are posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. The facility provides AM and PM snack, and lunch. Per Site Director, meals are prepared fresh daily at facility kitchen and serve in family style.

Medication: This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. A Plan of Operation for Incidental Medical Services was submitted on 01/11/18.

Staff and Children’s Records were reviewed for completeness; Inspection of required forms was made. LPAs issued the Confidential Names List (LIC 811) to the Assistant Director during this inspection. The Confidential Names List documents the staff and children’s files that were reviewed during this inspection.

LPA advised the licensee to access forms, effects of lead exposure flyer, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

No deficiencies were observed or cited in accordance with Title 22 California Code of Regulations on this date.



The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Arsine Aghazarian, Director including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
