25 | Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced annual inspection on 11/03/2022 at 8:50 AM. LPA met with Lorette Avanessian, Director. LPA provided Lorette Avanessian with a copy of the Entrance Checklist for Child Care Centers form (LIC 125). Director guided LPA on a tour of the facility. This is a preschool program which consists of 10 classrooms, Pink 1 and Pink 2 (2.5- 3 years old), Purple 1, Purple 2, and Purple 3 (3-4 years old), Blue 1, Blue 2, and Blue 3 (4-5 years old), Kindergarten 1 and Kindergarten 2, Library, and Auditorium/multi purpose room. Facility operation hours are from Monday to Friday from 7:30 AM to 5:30 PM.
All areas identified on the Facility Sketch were inspected. Upon arrival, the following staff were present during this inspection; Preschool Room: Pink 1: 15 preschoolers with 2 staff, Pink 2: 14 preschoolers with 4 staff, Purple 1: 12 preschoolers with 3 staff, Purple 2: 18 preschoolers with 2 staff, Purple 3: 16 preschoolers with 2 staff, Blue 1: 17 preschoolers with 2 staff, Blue 2: 13 preschoolers with 3 staff, Blue 3: 12 preschoolers with 2 staff, Kindergarten 1: 22 children with 4 staff, and Kindergarten 2: 17 children with 2 children. The facility was observed to be within the license capacity and limitations. Staff names were recorded. All children were observed to be under visual supervision of a teacher at all times. The following was observed during the tour of the facility:
PHYSICAL PLANT
LPA observed the facility to be clean, safe, sanitary and in good repair. Per director, facility genitors clean the facility daily after hours. Furniture and equipment was inspected for age appropriateness and good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All materials accessible to children were observed to be toxic-free. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Parents take Linens home each week on Friday or as needed and return it on Monday. Napping equipment (cots) were observed to be stored in each classroom. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed.
