Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced annual inspection to the above facility on 09/17/2021. LPA arrived at the facility at 8:40 AM and met with Malisa Smith, Director who guided analyst on tour of facility. This is a preschool program with Toddler option. Preschool program consists of 3 classrooms which are located on the lower level, RM 12 (3 years old, 2.5-3.5 ), RM 13 (4 -5 years old), RM 11 ( PreK 4.5-5), Toddler Option classroom room is located on the first floor. This facility is located on Glendale City Seventh/ Day Adventist Church property. Facility operation hours are from Monday to Friday from 6:30 AM to 6:00 PM. Per the Director there are 17 children enrolled.

All areas identified on the Facility Sketch were inspected. The following staff was present during this inspection: RM 11: No children were present at the time of the tour, RM 12: 1 staff with 3 preschoolers, RM 13: 2 staff with 4 preschoolers, RM 9: Empty, and Toddler room: 2 staff with 4 toddlers. The following was observed during tour of facility:

At approximately 8:50 AM LPA Anomeh Eivazian began facility tour with Director. All areas identified on the facility sketch were inspected. LPA observed 7 preschoolers with 3 staff and 4 toddlers with 2 staff. Teacher-child ratios were observed to be in accordance with Title 22 regulations. The Licensee is within the conditions, limitations, and capacity specified on the license. Staff names were recorded. All children were observed to be under visual supervision of a teacher at all times.



LPA observed the facility to be clean, safe, sanitary and in good repair. Per director, cleaning crew come daily to clean the facility. Furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean.

All toilets and hand washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, the office is used as an isolation area. There are chairs in the office. Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted.

Snack menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. Children enrolled bring their own lunch. The facility provides AM and PM snack. Preschoolers eat in lunch room which is located in the first floor adjacent to the kitchen, and toddlers eat in toddler classroom. All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. There are two kitchens in the facility that are being used for this programs, one kitchen which is adjacent to the office is being used for preschool program, and the church kitchen is being used for Toddler Option. All food is protected from contamination, and LPA inspected that any contaminated food is discarded immediately. There is drinking water available in all indoor classrooms and outdoors. Per director children use their water bottles and teachers take children water bottles in trays outside.

All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. Storage areas for poisons are locked. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. The licensee takes measures to keep the facility free of flies, other insects and rodents.

Napping equipment and bedding was inspected for good condition, appropriate storage and cleanliness. Storage for children's belongings was inspected. Linens are washed weekly in the facility. There is Laundry equipment in the lower level between two bathrooms. This area is inaccessible to children.



There are two separated outdoor playgrounds on the facility for preschool and toddler option programs. Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. During this inspection LPA inspected additional garden area as preschool outdoor play area. Garden area was observed to be in compliance with Title 22 Regulations. The Director states that there are no bodies of water on the premises and LPA did not observe any bodies of water during this inspection. Director states there are no weapons or firearms on the premises.

Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Medication: This facility provides Incidental Medical Services- IMS. Currently there is no children enrolled with IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm. Plan of Operation for IMS obtained during this inspection.

INFANT CARE: LPA advised director, Cribs or play yard shall not hinder the entrance or exit from the sleeping space, mattresses shall be firm and covered with a fitted sheet that overlaps the underside so it cannot be dislodged. Cribs and play yards shall be free of loose articles and objects. No objects shall be hanging above or attached to the side of the crib. LPA did not observe any infants swaddled while in care. LPA advised the Licensee that infants shall be placed on their backs for sleeping and shall be supervised. Infants shall be checked on every 15 minutes and the time of each 15-minute check shall be documented with child’s name and date. The LIC 9227 Individual Infant Sleeping Plan shall be completed for each infant up to 12 months of age. A copy of the LIC 9227 was provided to Licensee. LPA provided the licensee with a copy of A Child Care Provider’s Guide to Safe Sleep, by American Academy of Pediatrics. LPA also consulted and explained Child Abuse Reporting, Never Shake a Baby, and Safe Sleeping practices.

SAFE SLEEP: LPA discussed the safe sleep regulations with director, Melisa Smith and discussed the Child Care Licensing Safe Sleep webpage at http://www.cdss.ca.gov/inforescources/child-care-licensing/public-information-and-resoucrces/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at http://www/cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. During this inspection total of 11 children were present in the facility and all 10 children were signed in. All present Staff Records were reviewed to ensure that a health screening report is on file. 11 Present children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed. Last Disaster Drill was conducted on 08/11/2021 as read by LPA from disaster drill log.

The following was discussed:
- Reporting Requirements.
- The facility shall ensure that each child is accorded the following personal rights: to be accorded safe, healthful and comfortable accommodations.
- All personnel shall be in good health and shall be physically and mentally capable of performing assigned tasks; personnel that pose a threat to the health and safety of children shall be relieved of their duties.
- All personnel shall be given on the job training in housekeeping, sanitation principles and universal health precautions.

LPA advised the licensee to access forms, regulations, providers information notices (PIN), and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.



At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today.
