Licensing Program Analysts (LPA) Anomeh Eivazian and Staicy Perry conducted an unannounced required 1- year inspection on 09/15/23 at 12:50 PM. Director, Melisa Smith was not present during this inspection. LPAs met with Ligia Pomol, Toddler Teacher who contacted Director, Malisa Smith. Shortly after Satnick Farhadenejad, preschool teacher/assistant director arrived to the facility who guided analysts on tour of facility. This is a preschool program with Toddler option. Preschool program consists of 3 classrooms which are located on the lower level, RM 12 (3 years old, 2.5-3.5 ), RM 13 (4 -5 years old), RM 11 ( PreK 4.5-5), Toddler Option classroom room is located on the first floor. This facility is located on Glendale City Seventh/ Day Adventist Church property. Facility operation hours are from Monday to Friday from 6:30 AM to 6:00 PM. Per the Director there are 9 children enrolled.

All areas identified on the Facility Sketch were inspected. The following staff was present during this inspection: RM 11: No children were present at the time of the tour, RM 12: no children, RM 13: 2 staff with 3 preschoolers napping, RM 9: Empty, and Toddler room: 2 staff with 3 toddlers napping. The following was observed during tour of facility:

At 1:15 PM LPAs began facility tour with teacher Satnick Farhadenejad. All areas identified on the facility sketch were inspected. LPAs observed 3 preschoolers napping in room 13 with 2 staff and 4 toddlers were napping in toddler classroom with 2 staff. Teacher-child ratios were observed to be in accordance with Title 22 regulations. The Licensee is within the conditions, limitations, and capacity specified on the license. All children were observed to be under visual supervision of a teacher at all times.



PHYSICAL PLANT
LPAs observed the facility to be clean, safe, sanitary and in good repair. Furniture and equipment was inspected for age appropriateness and good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All materials accessible to children were observed to be toxic-free. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are laundered by the facility

each week on Friday or as needed. Napping equipment (cots) were observed to be stored in each classroom. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Per teacher Farhadnejad, cleaning crew clean the facility daily after hours. Availability of indoor drinking water was observed in the classroom. Per teacher Farhadnejad children use their water bottles for drinking water and they refill it by kitchen tap water or outside fountain water.

Disinfectants, cleaning solutions, medication, and other items that are dangerous to children, were inaccessible to children. Teacher Farhadnejad states at 1:45 PM, there are no poisons stored at the facility. Teacher, Farhadnejad tested the carbon monoxide detectors in the lunch room/multi-purpose room. It sounded off the alarms and heard by LPAs to be functional. LPAs observed required posting information were posted in entrance hallway.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. There are two kitchens in the facility that are being used for this programs, one kitchen which is adjacent to the office is being used for preschool program, and the church kitchen is being used for Toddler Option. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each meal.

There are two separated outdoor playgrounds on the facility for preschool and toddler option programs. Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with rubber material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed. Per Teacher Farhadnejad children use their water bottles and outdoor water fountain for drinking. LPA advised that no children shall be left without the supervision of a teacher at any time.

The Teacher Farhadnejad there are no bodies of water on the premises and LPAs did not observe any bodies of water during this inspection. Per Teacher Farhadnejad, there are no weapons or firearms on the premises.

FACILITY RECORDS
Teacher Farhadnejad was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Nine Children’s Records were reviewed for completeness; Inspection of required forms was made and documented on the LIC 857. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. LPAs also reviewed four staff records. The review of Staff records was documented on the LIC 859. Staff present had proof of the AB 1207 Mandated Reporter Training certificate on file. Staff present had proof against TB, measles, pertussis, and influenza. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions.

Children's roster was reviewed and is current. Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. LPAs observed sign in/out sheet was placed in the hallway outside the office area. During this inspection 7 children were present, and all 7 children were singed in. Disaster drill log was available, last drill was conducted on 07/14/23.

Menus are posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. Preschoolers and Toddlers are provided AM and PM snack. Children enrolled bring their own lunch. Preschoolers eat in lunch room which is located in the first floor adjacent to the kitchen, and toddlers eat in toddler classroom.

First Aid supplies were observed in the classroom. According to teacher Farhadnejad, medication is only administered to a child when accompanied with a doctor's note and is stored in the kitchen by the director office. The facility was observed to be equipped with an isolation area for any child who becomes ill and it is located in the office. The ill child will use the family restroom which is located in the hallway of first floor during isolation which is disinfected immediately after use per Site Supervisor.

LPAs discussed the safe sleep regulations with teacher Farhadnejad and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPAs also informed teacher Farhadnejad of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Medication: This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226.The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm. Plan of Operation for Incidental Medical Services on file. During this inspection LPAs observed Child#3 Epi-Pen was expired on 09/2022. During this inspection teacher Farhadnejad contacted child#3 mother who stated child#3 still is in need to use Epi-Pen and she was notified by the school to bring a new Epi-Pen. Per child#3 mother she is going to bring a new Epi-Pen to school on Monday 09/18/23.

Analyst reviewed new Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, which requires the testing of water for lead in Child Care Centers (CCCs) with facility director during the inspection. Per AB 2370, all CCCs that are located in buildings constructed before January 1, 2010, must have their water tested and post the results by January 1, 2023, and every 5 years after the date of the first testing. LPA Eivazian talked to Director, Malisa Smith over the phone who stated they completed Water Lead Testing late in 2022 and will submit LIC 9275, LIC 9276, and labeled facility sketch to LPA by 10/16/23.

LPA Eivazian informed teacher Satnick Farhadnejad that this report dated 09/15/23 documents 1 Type A citation. Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Eivazian informed the teacher Satnick Farhadnejad to provide a copy of this licensing report dated 09/15/23 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Teacher Satnick Farhadnejad was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
