Licensing Program Analyst (LPA) Crystal Green conducted an unannounced 1 year required inspection. LPA met with designated facility Director, Michael Barkyoumb, also present during the inspection was, April Stover, Senior Director of Programs. Due to COVID- 19 precautionary measures were taken, individuals present during inspection wore appropriate personal protective equipment. Facility was observed to be adhering to local public health guidelines.

At approximately 10:15 AM LPA Green began inspection of the school-age program. All areas identified on the facility sketch were inspected. This facility also services an adult day program also licensed through the Department of Community Care Licensing #198602059. Per Director, the adult program conducts their daily activities separately from the school-age program however there are shared spaces that are utilizes by both programs at staggered time to avoid commingling. LPA was provided a tour of the areas designed used by the adult program and areas designed for used by the school-age program.

The facility is currently closed as a COVID-19 precautionary measure. Per Director, children have been attending the facility since April 2020 virtually through Zoom. This facility recently completed renovations of the indoor and outdoor areas. No changes have been made to alter the capacity granted to the licensee. Areas used by children that were recently renovated; the restrooms, kitchen, classroom, and outside walkway leading to outdoor space. LPA observed (4) restrooms that recently renovated providing additional space within each restroom. All toilets and hand washing facilities are in safe and sanitary operating conditions. Per Director, when the students return they will be assigned a specific restroom to utilize based on cohorts as a COVID precautionary measure.

Due to COVID- 19 precautionary measures, Director states that student drop off and pick up will be conducted outside in the parking lot prior to the child leaving the vehicle. Per Director, wellness and temperature checks will be conducted at the vehicle before the child is allowed entry. All visitors must enter the facility through the main entrance. Per Director, temperature and wellness check will be conducted prior to entry into the areas accessible to the children. This facility uses Dr. Owl application to conduct the wellness check of both visitors, staff, and the participants. Report Continues Page 1 of 3

LPA observed the facility to be clean, safe, sanitary and in good repair. Furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. LPA observed plastic divider place on each table to as a COVID-19 precautionary measure and tables were observed to follow social distancing guidelines.

All materials and surfaces accessible to children are toxic free. At this time, the isolation room will be located next to the restrooms. There are chairs in the designated isolation room. Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted.

Snack menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. The facility will provide AM snack and PM snack. As a COVID-19 precautionary measure, lunch will be provided in prepacked container or lunch bag with labels included. All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. There is drinking water available in all indoor classrooms and drinking containers / jugs are taken outdoors. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. Storage areas for poisons are locked. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. The licensee takes measures to keep the facility free of flies, other insects and rodents.

Outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. Outdoor activity space surface is maintained in a safe condition as is free of hazards. LPA observed the facility to have a designated walking path leading to their outdoor space surrounded by an 8-foot-high fence to prevent unauthorized access to the outdoor area. LPA also observed a patio area with picnic table designated for school-age children that provided adequate shade. Per Director, there was a pool previously located in the new patio area however LPA did not observe any bodies of water during this inspection.

Staff Records were reviewed to ensure that a health screening report is on file. Children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.
