On 05/02/2024 at 10:00 AM an Unannounced Required – 3 Year Inspection was conducted by Licensing Program Analyst (LPA) Katrina Chicote to the above facility. LPA announced purpose of visit and initially met with Facility Representatives (FR) Denise Martin, Office Secretary, who allowed entry to facility and provided guided tour of the facility both indoors and outdoors. Director, Christa Dunlap, arrived after tour was conducted. Facility is currently licensed for a capacity of 60 and per FR they currently have 55 enrolled. The program currently operates three programs with varying hours: Full Day Program Monday thru Friday from 7:30 AM to 5:30 PM, Extended Day Program Monday thru Friday from 9:00 AM to 3:00 PM, and Morning Preschool Program Monday thru Friday from 9:00 AM to 11:30 AM. LPA observed facility license, LIC 610, Parent's Rights, and Personal Rights posted at time of inspection. LPA discussed posting requirements.

The facility is located on the California Statue University Northridge (CSUN) campus and is made up of three classrooms named for their scheduled program times: Full Day, Extended Day, and Morning Preschool. At the initial start of the site visit, LPA observed 16 children with three staff (S1, S2, and S3) in Full Day, 16 children with four staff (S4, S5, S6, and S7) in Extended Day, and 15 children with four staff (S8, S9, S10, and S11) in Morning Preschool, total census of 47 children and 11 staff present. LPA observed additional student volunteers from CSUN campus, per FR these student volunteers are here to complete field work for the college and stay a maximum of 3 hours.

Beginning at 10:06 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. Required shade and fencing were inspected. LPA observed surfaces of outdoor play area to be sandboxes, asphalt, and grass that are maintained for the activities planned at time of inspection. LPA observed a play structure with climbing equipment, slide, and monkey bars that has sand below as cushioning material below to absorb fall. LPA observed corner of play yard to have plastic dramatic play houses and kitchen not maintained at time of inspection that is piled with sand on top with cracks, broken ends and plastic pieces broken off. LPA observed bicycles and scooters available for children not maintained

that were rusted and had handle bars missing and some with taped up handle bars at time of inspection. LPA observed a sensory archway that was leaning with metal bar bent in play yard at time of inspection. LPA observed wooden play structures available for children as climbing and play equipment that was maintained at time of inspection. LPA observed child appropriate seating through out the outdoor play area and a variety of storage barns/sheds for additional play equipment. LPA observed there is a small building in middle of outdoor play area with books, table, and chairs. Per FR, this room is used for children receiving one-on-one services. LPA advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to a drinking fountain during outside play. LPA observed additional front outdoor play area that has grass through out and plastic play equipment that were maintained at time of inspection. Per FR, children mostly use back outdoor play area but will use the front yard for outdoor play when it is close to pick up time. Play area was inspected for inaccessibility to bodies of water and no bodies of water were observed at time of inspection.

Beginning at 10:15 AM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPA observed surface of classrooms to be safe and clean, and child size tables and chairs available for children well maintained. LPA observed a variety of age appropriate toys available for children used for dramatic play, building, math, and art that are well maintained free of loose and sharp parts at time of inspection. LPA observed areas with seating and quiet play available in each classroom. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Cots were inspected. LPA observed cots to have individual child's names labeled and to be laid out as to not inhibit walkways and entrances/exits at time of inspection. Per S4, linens are provided by the parents and taken home weekly for washing. Availability of drinking water was reviewed. Per Staff, children bring in their own water bottles that are refilled by water faucets that are filtered in classrooms. LPA observed personal water bottles and operable water faucets in classroom at time of inspection. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPA observed bathrooms available for each classroom that each had three toilets and three sinks with additional sinks in the classrooms.

Snack/lunch menus were not posted. Per FR, facility does not provide snacks and lunch and children bring in their own snacks and lunches from home. LPA observed a cart with lunch boxes with children's names labeled at time of inspection. LPA discussed requirements for AM and PM snacks at time of inspection.

A first aid kit is kept in lobby which LPA observed and inventoried for necessary supplies. Per FR, additional first aid kits are stored in each classroom. At 10:34 AM, LPA tested carbon monoxide detector in Morning Preschool Room and observed to be in operable condition at time of inspection. LPA observed smoke detectors and fire extinguishers are present in the facility. Disaster drill log was available and last drill conducted on 04/11/2024. Per FR, sick/isolation area for sick children is in lobby or Director's Office. LPA observed medicines stores for three children (C1, C2, and C3) stored inaccessible to children at time of inspection. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. LPA observed these items stored on top of refrigerator in off limits staff only room that is kept locked with a high latch. LPA observed this room to be locked at time of inspection.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPA observed all children present signed in at time of inspection. Personal Rights of children were observed by LPA.

Beginning at 12:30 PM, staff and children’s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. Criminal Record Clearances were reviewed for adults. CPR card expires on 08/2025 for S10. 5 of 6 staff files reviewed did not have proof of the AB 1207 Mandated Reporter Training certificate on file.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. CCC DID NOT COMPLETE TESTING PRIOR TO THEIR DEADLINE:
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

The following were discussed with the Director:

Licensee (or facility representative) was reminded that all adults 18 and over in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

AB 1207: Beginning on January 1, 2018, this law requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Volunteers are encouraged but not required to take the training. Website: http://www.mandatedreporterca.com/

LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation. This includes any changes that should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.



LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

Based on this information, the following deficiencies on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

A notice of site visit was given and must remain posted for 30 days.
