Licensing Program Analysts (LPA) Susann Sanchez conducted an unannounced annual required inspection at the above facility on at 1:30 PM. LPA met with Director Jennifer Kim, guided LPA on a tour of the school aged center. School Age program operate 2:50 PM - 7:00 PM, Monday - Friday. The facility is adjacent to West Hills Preschool (License #191227976). Each facility has its own entrance. The director's office is located in the preschool side. The ill/isolation area shall be in the director's office.

Director states that there are no weapons or firearms on the premises. LPA did not observe swimming pools or spas on the premises. There are age appropriate toys and equipment on the premises.

At 1:45PM, Director stated that facility has not done the required lead testing. Type A was cited.

At 2:00PM, Adult arrived at the facility who is not fingerprint cleared. Type A cited.

At 2:20PM, LPA observed that the carbon monoxide is not working and did not see a smoke detector. Type B was cited.

LPA toured the School Age classrooms and outdoor play area. Furniture and equipment was inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Storage for children's belongings was observed. Availability of drinking water was reviewed. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. Materials for the children were observed to be clean. A first aid kit is kept in the classroom. Fire extinguishers have been serviced. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. Children are provided an afternoon snack.



Outdoor equipment was inspected for safety, cushioning material, good repair and appropriateness. Required shade, drinking water and fencing were inspected. LPA Sanchez advised that the children need to be within the direct care and supervision, including visual supervision of the teachers at all times.

Children’s Records were reviewed for LIC 700- Identification and Emergency Information, LIC 702- Child's Preadmission Health History, LIC 627- Consent for Medical Treatment, and LIC 995A Notification of Parents’ Rights. Sign-in and out procedures were reviewed.

Staff records were reviewed for LIC-501: Personnel Record, LIC 508-Criminal Record Statement, LIC 9052- Employee Rights, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse and current Mandated Reporter Training Certificate. All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.



Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.



Facility Representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Consult was given regarding TSP.

The notice of site visit was posted where the parent/guardian of children enter and exit the facility. A copy of this report shall also be posted where the parent/guardian of children enter and exit the facility. Both the notice of site visit and licensing report shall remain posted for 30 consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon their return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled child for the next 12 months. A signed Acknowledgement of Receipt (LIC9224) shall be in each child’s file, acknowledging receipt.

Based on the LPA’s observations, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22 Chapter 1 and Section CCR & H&S. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

A notice of site visit was given and must remain posted for 30 days. Appeal Rights were given. Exit interview conducted and report was reviewed with the Jennifer Kim.
