On 01/09/2023, Licensing Program Analyst (LPA) Suzette Ornelas conducted an unannounced Annual Required Inspection for the facility. LPA met with Jennifer Guevara, Director and toured the facility indoors and outdoors. Center has preschool license (191290429) on the premises. School age Children and preschool children do not co-mingle. LPA observed 0 children and 0 adults. Days and hours of operation are: Monday, Wednesday, Thursday from 2:30pm to 8pm and Tuesdays from 1:30pm to 8pm. School is closed on Fridays for school aged children.

Center follows the LA Valley College year round campus schedule.

Children Sign-In According to the Site Director:
Sign in and out sheets were not reviewed due to no school age children present. Children are dropped off/picked up by their parents. LPA reminded director that parents must sign children in/out.

RM 5/RM6: (5y-13y)
All areas identified on the Facility Sketch were inspected. The facility consists of a total of 6 classrooms. The school age program operates in rooms 5 and 6; however, due to current enrollment, only room 6 is being utilized. There is a small kitchen space between room 5 and 6 which is inaccessible to children in care by the use of a small door

LPA observed all required postings in a prominent area. All children are under supervision, including visual supervision, of a teacher at all times. Capacity and limitations as specified on the license are being maintained. A walk through of the classroom area was conducted. Classroom space was found to be clean and free from any potential hazards. Furniture was found to be in good repair and age appropriate. There is adequate heating, lighting and ventilation. LPA observed classroom space to have arts/crafts, reading, individual cubbies for storage and learning activities. There is a first aid kit located inside each classroom.

LPA inspected and observed two bathrooms inside room 6. Boys and girls can use either bathroom one at a time. Each bathroom has 1 toilet, and 1 sink for the school age children. Toilets and sinks are functioning properly and are age appropriate. LPA observed soap, toilet paper and paper towels readily available. Water temperature was tested and is appropriate.

Children play in the yard located right behind room 6. The outdoor play yard is fenced and is separated from the pre-school yards. The after school program play yard was inspected during this visit for health, safety, good repair and age appropriateness equipment. The area was observed to be free of debris. Play area was inspected for hazards and inaccessibility to bodies of water. LPA explained to the director to ensure that outdoor supervision is exceptional due to items that are against the fence, because school age children may climb the items that are against the fence. LPA explained that this is to prevent any school age child from wandering away or getting on the other side of the fence which is directly a parking structure and a main busy street.

All food preparation and storage areas are clean and safe. Per Director, program does provide daily meals. There is a clean, fully equipped kitchen with stove, refrigerator, freezer and microwave oven. The facility provides an afternoon snack and dinner for the after school age children. Menus are posted weekly. Currently, there are no children with allergies enrolled in this program, however if there are children with allergies, the list will be posted on the refrigerator. LPA observed an appropriate amount of food and snacks.

Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is readily available both indoors and outdoors. The facility is equipped with a working smoke detector and carbon monoxide detector in all classrooms. LPA observed multiple fire extinguisher (2A 10BC) located on the campus serviced 1/2022. LPA explained to director to ensure it is serviced by the end of the month. There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. No poisons were observed during the inspection.

LPA reviewed a sample of children’s files and observed files were complete. Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at off site activities. The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. LPA reviewed a sample of staff files and observed files were complete.

Children are inspected for illnesses as they arrive. A review of medication policy indicated the facility does not dispense medication. Children's parents come to the facility to dispense the medication to the children. The Director's office is utilized for isolation and care of ill children. There is a cot available for each ill child. The bathroom located on the reception area is utilized for ill children.

Incidental Medical Services (IMS) are not currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. Information regarding Americans with Disability Act (ADA) can be obtained by contacting US Department of Justice toll free ADA Information line at (800) 514-0301(voice), (800) 514-0383 (TDD) and website link https://www.ada.gov/childqanda.htm.

LPA and Licensee discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies are being cited.

Licensee was provided a copy of their appeal rights. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
