On 10/19/2021 Licensing Program Analyst (LPA) Shandra Powell conducted an unannounced inspection with Lenita Exner, Licensee also present was Staff #1 (assistant, daughter of licensee) for the purpose of an Required 1-Year inspection. The purpose of this inspection is to ensure the standards for a Family Child Care Home are being met in accordance to California Title 22 Regulations and California Health and Safety Codes. Per licensee the ages of the children in care are 3 months to 10 years of age. The days and hours of operation are Monday thru Friday from 7am to 6pm. Licensee was informed that any changes to ages, hours and days of operation shall be submitted to the department for approval prior to initiation of changes. Per Licensing Information System (LIS) all adults residing and working in the home have obtained background clearances.

This is a one story home which consists of 5 bedrooms, 4 bathroom, (2) kitchen, dining room, living room, front yard and backyard (fenced) with converted garage (Day Care Room). Entire home and backyard is OFF-LIMITS to children in care. Licensee confirmed. The licensee states there are no weapons at the facility. LPA observed a ring camera at front door of home. Child care is provided in Day Care Room (converted garage) and children have access to the bathroom adjacent to kitchen #2 which is adjacent to Day Care Room.

Furniture and equipment were inspected for cleanliness, age appropriateness & good repair. Napping & Bedding equipment were inspected for cleanliness & good repair. Bathroom facility was inspected for inaccessibility of toxins and chemicals. First- Aid Kit was observed by LPA and is inaccessible to children in care. LPA observed a smoke detector located in the kitchenette area near the children's playroom which was found to be in working order. LPA also observed a Carbon Monoxide detector in working order.

The Fire Extinguisher is kept in the kitchenette area inaccessible to children in care. Kitchen area was inspected for inaccessibility of toxins/cleaning compounds and other potentially dangerous objects/materials. The Back yard play area was inspected for safety, potential hazards & inaccessibility to bodies of water. The Back yard is fully fenced. There is a pool in the back yard that is fully fenced and inaccessible to children in care. LPA observed eight doors leading from home to pool with Iron bars placed on the outside of the doors (photos taken) all doors remain locked during child care hours confirmed licensee. There is a self-closing self-latching gate that opens away from the pool area. LPA observed bolt key lock on gate but was not locked during initial inspection. LPA advised licensee to keep gate locked at all times. Children's play yard is to the right of the home. Toys and play equipment are age appropriate. LPA observed two bikes on the play yard .that are missing the rubber handles on the bike handle bars. This poses potential health and safety risk to children in care LPA advised licensee to replace rubber handles or remove from play area until in good repair.
LPA was shown the Children's records LPA reviewed for completeness however no Children files were complete both files were missing children immunizations. Facility Roster, Emergency Disaster Plan and Disaster Drill logt Licensee and Assistant records were reviewed for completeness however the Licensee and Assistant were out of compliance for Immunization's and current pediatric CPR and First Aid Certification (CPR expired 04/2019) and Mandated Reporter Training this poses a health and safety risk to children in care.
LPA discussed the safe sleep regulations with Applicant LPA observed Safe Sleep Log for infant during inspection:https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) are not currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. The home has working telephone service and LPA confirmed the phone number is (818) 360-6597. There are currently one infant in care. LPA discussed Safe Sleep Regulations with licensee. There is two play yards. LPA discussed with Licensee play yards are kept free from all loose articles and objects while infants are sleeping, and there are no objects hanging above or attached to the crib or play yards. Infants are not swaddled while in care. Provider physically checks on sleeping infants every fifteen minutes and documents any signs of distress which includes but is not limited to flushed skin color, increase in body temperature, restlessness and labored breathing. Infants can be visually observed through an open door if sleeping in a separate room. Individual Infant Sleeping Plan is completed and in file for each infant up to 12 months of age. Infants up to 12 months of age are placed on their backs for sleeping.

Licensee ensures that children in care are supervised at all times and is aware children shall not be left in parked vehicles. Car seats are used for transportation purposes only and are not used for sleeping children. Capacity as specified on the license is being maintained.

LPA reviewed a sample of children’s files and observed files were incomplete with emergency information as required. Licensee’s nor assistant Mandated Reporter Training was not completed this poses a health and safety risk to children in care. A review of records indicates that all employees and/or volunteers have no immunization records on file for influenza, pertussis and measles this poses a health and safety risk to children in care.

Immunizations: Commencing September 1, 2016, SB 792, prohibits a person from being employed or volunteering at a childcare facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the inspection.

Mandated Reporter Training:
Beginning on January 1, 2018, AB 1207, requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years.
