Licensing Program Analyst (LPA) Bardo Baluyot conducted an unannounced required annual inspection. LPA also conducted this visit as Maryvale provides services to military families. According to Director, Crystal Reynolds, licensee has 1 child of military families enrolled at this time Maryvale also has an infant/toddler program which is licensed through the Department ( #1980096580). The preschool and infant programs do not have any shared areas.

LPA inspected areas according to the facility sketch on file. No alterations have been made to the exterior or interior of the facility. Four classrooms are used for care; Room #1 (Jelly Fish), Room #2 (Star Fish), Room #3 ( Dolphins) and Room # 4 (Penguins). Currently, Dolphins classroom is not being used due to low enrollment. Restrooms are located between each preschool room. There is an additional restroom available for children to use. Children are signed in via a fingerprint scanning system. A back up sign in and sign out book is also available should there be a power outage. LPA observed a total of 23 children present during inspection.

Furniture and equipment was inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings and cots and blankest are stored appropriately. The isolation restroom is located across the Director's office. General sanitation was observed. Availability of indoor drinking water was also observed (jugs and water cups available for children).

The playground consists of a sandbox, mud kitchen, grass and hard yard areas. No hazards were observed. Water fountains were observed outdoors and in operable condition but are not in use due to the pandemic.

Per Director Reynolds, all meals are prepared on site. LPA toured the kitchen which is located on the opposite side of the preschool. Kitchen observed to be clean and well staffed. Food and snack menu was reviewed for availability and appropriateness to children in care. Licensee offers breakfast, lunch and one snack. Menus observed to be posted along with all other posting requirements. Facility conducts routine disaster drills monthly (last conducted on 7/29/2021 ) and records are kept with Maryvale's facilities Department. An operable carbon monoxide detector was observed as well.

Teacher child ratios were observed to be in compliance. Care and supervision was evaluated to determine if the basic needs are met. Children's records were reviewed for completeness. New employee records were reviewed as well. All employees have current immunization records on file. Mandated Reporter Certificate on file for all staff. First Aid Kit inspected and verification of current CPR/First Aid for staff was observed to be current (expires 2023).

Exit interview conducted. Appeal Rights explained. Notice of Site Visit must be posted for (30) days. Failure to do so may result in a $100.00 civil penalty.
