25 | Licensing Program Analyst (LPA) Nolan Tcheng conducted an unannounced Case Management inspection to follow up on an incident report submitted to the department on 05/04/2023. Upon arrival at 1:58pm, LPA was greeted by Director Marie Ostrowski, to whom the purpose of the inspection was explained. At 2:05pm, LPA was provided a tour of the facility where there were children present during the time of inspection.
Census was taken. There were 22 children and 3 staff members.
On 05/04/2023, director was approached by the parent of a child enrolled in the program. There was concern from the parent regarding their child. Facility self-reported to the department within the required 24hrs from being made aware. Per director, parent has had a meeting with the facility to explain and share the concerns and information reported to the Department. Facility conducted internal investigation and discussed with the parent that there was no information disclosed regarding their concerns. Per director, Parent did not make claim that the facility was the cause of their concern.
During the inspection, LPA interviewed director. Pictures and emails related to the incident were obtained during the inspection.
Per California Code of Regulations Title 22, Division 12, no deficiency cited during today's visit.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
