1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	Licensing Program Analyst (LPAs) Cynthia Reyes and Carolyn Tuba, conducted an unannounced 1 year annual inspection on this date. LPAs met with Assistant Director, Jennifer Whiteside and a copy of the Entrance Checklist for Child Care Centers form (LIC 125) was provided. A Covid-19 risk assessment was conducted prior to entering the facility. Due to COVID- 19 precautionary measures were taken during the entire inspection and all individuals present during this inspection wore appropriate personal protective equipment. There are a total of 4 classrooms designated for the preschool program. Hours of operation are M-F 8am-12pm. This is a morning program only. 9:15 AM The tour began at the front lobby area where LPAs were guided into the first classroom called the Big Room. LPAs observed that parents utilize an App System called Brightwheel and there is a Waiver on file. Temperatures and health checks will be conducted. Use of hand sanitizer will also be available. Parents do not enter the classrooms and facility staff will come to the door and walk the child inside the classroom and bring the child out at pick up time. LPAs observed that all licensing forms and Covid-19 posters were located by the Director's office and it was stated that they have a central point located outside due to Covid-19 by both entrances that are used to drop children off. Facility was observed to be adhering to local public health guidelines. (According to CDC guidelines, children under the age of (2) are not required to wear masks.)

All areas identified on the Facility Sketch were inspected. The following was observed during the tour of the preschool classrooms. LPAs observed in Big Room there were 2 teachers and 1 support staff with 23 children. Room A had 1 teacher with 10 children. During our tour Director, Vicky Gutzwiller joined us during our inspection of classroom B to introduce herself. In Room B there was 1 teacher and 1 aide with 6 children. Room C had 1 teacher and 9 children. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. All children were observed to be under supervision and observation, including visual supervision, of a teacher at all times. The licensed facility is within the conditions, limitations, and capacity specified on the license.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Preschool children have their own cubby to store their personal belongings. Staff restrooms are located in the main hallway, located off of the Big Room and is kept latched, when not in use.

9:30 AM The outdoor playground was inspected and the play equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade provided by big trees in the play yard. LPAs did not observe any bodies of water on the premises. The Preschool play yard is separated by gates for the main play yard and the blacktop area. However it was observed that in the main play area near the structure there is a purple gate that needs to be replaced as it has sharp broken edges. It is suggested that the playground be skimmed and cleaned each morning before children go outside each morning. LPAs observed an old water bottle by the bench and cob webs on play toys and dust. LPAs advise to clean and clear cob webs. PAGE 2 OF 5

Snack menu was reviewed and LPAs advised it needs to be posted on parent board. The facility provides am snack only and lunch is brought from home. There is drinking water available indoors and outdoors with water jugs with disposable cups.

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were observed to be inaccessible to children. It was advised that in Room C teacher left bag of her personal belongings and that staff should have a cabinet to stow away in a high place. Jennifer states there are no poisons located at the facility. Carbon monoxide and smoke detectors were observed and are operable. Jennifer stated there are no weapons or firearms on the premises.

Jennifer was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Children’s Records were reviewed for completeness; Inspection of required forms was made and documented on the LIC 857. LPA also reviewed Staff record, which were documented on the LIC 859. Staff present had proof against TB, measles, pertussis, and influenza, as well as current Mandated reporter.

There is at least one person trained in CPR and Pediatric First Aid present during this inspection. Pediatric CPR & First Aid is documented for staff who have it and is current for the Assistant Director.

Update: Effects of Lead Exposure and the following Provider Information Notices (PINs) Required Lead Testing: PIN 20-01 CCP Effects of Lead Exposure, and Guardian User Account Access - PIN 20-20-CCLD. were discussed.

Medication: This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Children's roster was reviewed and is current and number of total enrolled at this time was documented. Sign-In and Sign-Out sheets were reviewed. LPAs observed that 3 children were signed in by the Director, Jennifer Whiteside and they were advised that it needs to be the legal adult of the child sign-in and out each day. Disaster drill log was available and posted visibly for all to see. The last drill was conducted on 12/15/2021

First Aid supplies was observed in the director's office and Director stated that each classroom stores a First Aid.

LPAs observed trash cans throughout the preschool require lids for both inside classrooms and bathrooms.
