Licensing Program Analyst (LPA) Mireya Garcia conducted an unannounced annual random inspection visit. LPA met with Marcia Cagigas, Director, who guided analyst on a tour of the facility. This is a preschool program which consists of 6 classrooms; RM 108, RM 110, RM 115, RM 117, RM 118 and RM 120. Census was taken. Facility operation hours are Monday to Friday from 7:45 AM to 4:00 PM.

All areas identified on this report were inspected. Upon arrival, the following staff were present during this inspection: Room 108: Staff #1, #2, #3, #4 and #5 with 13 preschoolers; Room 110: Staff #6, #7, #8 and #9 with 16 preschoolers; Room 115: Staff #10, #11, #12, and #13 with 13 preschoolers; Room 117: Staff #14, #15, #16, #17, #18, #19 and #20 with 15 preschoolers; Room 118: Staff #21, #22 and #23 with 11 preschoolers; and Room 120: Staff #24, #25, #26 and #27 with 14 preschoolers. The following was observed during the tour of the facility:

PHYSICAL PLANT


Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are washed weekly by the center. Napping equipment (mats) were observed in a separate storage room in each classroom. LPA did observe that in RM 110, RM 115, RM 117 and RM 118 were missing electric outlet covers during this inspection. Per Director the isolation area is located in the office. In the office a mat can be placed on the floor for an ill child and the staff restroom is readily available if an ill child requires it. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. LPA observed that the facility has a carbon monoxide detector. General sanitation was observed. Availability of indoor drinking water was observed in each classroom.

The following deficiencies listed on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22.

Exit interview was conducted with Marcia Cagigas, Director and Jacqueline Zavala, Assistant Director, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

Per Director, a staff training/meeting will be held to discuss with staff that electrical outlets are being covered after uncovering them for use and the storage of disinfectants, cleaning solutions, and other items that could pose a danger if accessible to children. Director will provide LPA with a staff agenda/sign in sheet via email by POC due date 11/05/19.

Staff and Children’s Records were reviewed for completeness. Inspection of required forms was made. LPA also issued the Confidential Names List (LIC 811) to the Director during this inspection. The LIC 857 and the LIC 811 documents the staff names and children’s files that were reviewed during this inspection.

Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in. Disaster drill log was available, last drill was conducted on 10/17/2019.

Weekly food Menus are posted where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Lunch was reviewed for availability, quantity and appropriateness to children in care. Preschoolers are provided with breakfast, lunch and pm snack.

First Aid supplies were observed inside each classroom. According to the Director, medication is only administered to a child when accompanied with a doctor's note and is stored in the locked cabinet in the Teacher’s office.

Medication: This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA advised the Director to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. UPDATE: Parent’s Rights Poster (with complaint Hotline) and Car Seat Law changes was provided to the Director during this inspection. LPA advised Director of the Mandated Reporter Child Care Training AB1207 that became in effect January 1, 2018, and to maintain a copy of the certificate on file for each staff.

Disinfectants and cleaning solutions (Awesome cleaners, Detergent, conditioners) were observed to be stored in Room 110 underneath children’s sinks located where is accessible to children (Pictures were taken). Cleaning solutions were immediately removed by Director and stored in an inaccessible area. Director Marcia Cagigas states that there are no poisons stored at the facility.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. There are no trash cans with tight fitting lids in the classrooms. However, Director states that trash is removed from the classroom right after meal times.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. However, the outdoor area needs to be cleaned. The sand area is not kept clean (LPA took pictures) per Director a service call will be place to the facilities department and she will work with staff to ensure the outdoor yard area is kept clean and orderly. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed.

FACILITY RECORDS

All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. All staff have received an active criminal record clearance as a condition of their employment with the Los Angeles Community College District.

Children’s Records were reviewed for completeness; Inspection of required forms was made. LPA issued the Children’s Record Review (LIC 857) to the Director during this inspection. In review of children’s records, files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.
