32 | Staff disclosed the child was able to access the empty "yellow" classroom due to the restroom being accessible from both classrooms (Orange/Yellow). The door leading to the restroom from the "orange" classroom is left open so children can freely access the restroom.
Based on LPA review of documentation and interviews conducted, declarations obtained during today’s inspection, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California Code of Regulations, Title 22, Division, 12, Chapter 1, Article 06, Section 101229(a)(1) Care and Supervision, relating to this allegation have been cited on 08/14/2019 on LIC 9099-D.
Exit interview was conducted with the Director, Marcia Cagigas. The Director was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these forms.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
