On October 20, 2023, Licensing Program Analyst (LPA) Carolyn Tuba conducted an unannounced required 1-year inspection. A COVID-19 risk assessment was conducted. LPA entered facility through the main gate of the Flanner Head Start programs and headed towards the administration office. LPA met with Program Director, Rosie Ducoing. Director led LPA on a tour for the annual inspection and LPA took a census of 27 children and 6 staff in care. There are 5 classrooms that the program operates as a Head Start preschool program, which includes a Toddler component.

Hours of operation: The days and hours of operation for the Preschool Head Start vary by program: Monday-Friday, Classrooms B, #2 and #3 - 8:00 A.M.-1:40 P.M. Room #1 - 8:00 A.M. – 3:30 P.M. Room A – 8:00 A.M.- 6:00 P.M.

Location: Classrooms #1, #2 and #3 are enclosed by a chain link fence near the administrative offices. Classrooms A and B have a separate entrance surrounded by a chain link fence located north-west from the administrative offices. In order to gain access to the facility the administrative offices buzz the public inside the facility. The facility grounds have several gates to come on campus. There are 2 playgrounds near classrooms #2 and #3. One playground is for the Toddlers and the other is for the preschool age. Classrooms A and B have their own playground located behind the classrooms. All the playgrounds are surrounded by a chain link fence. Currently classrooms #2 and #3 are not operating.

All posting requirements were observed on the Parent Board within each portable. The Parent Board is located on the wall adjacent to the door through which the children will enter. A monthly menu is posted on the parent board inside each classroom. All food is provided by the School District. Breakfast is served at 8:00 am, lunch at 11:00 am and P.M. snack at 1:20 P.M.

Classrooms #1, #2, #3 currently have a mini fridge, with microwave and adult sink. Classrooms A and B

have a kitchen/work area which is located between both classrooms and a half door separates each side to keep children safely out of the area. There is currently one (1) child who requires medication and are kept in the staff’s emergency backpack, which is kept on a hook inaccessible to children in care. If medication requires refrigeration there is a mini fridge available. There is a nurse on site who will train staff on how to administer medication. First Aid Kit is kept in the staff emergency backpack.

There is a carbon monoxide and smoke detectors that are connected to Willow district office and are tested by an independent alarm company through-out the year and as needed. The last annual maintenance on the Fire Extinguisher was on July 12, 2023. Classrooms also have a plugged-in carbon monoxide detector that was tested and operable.

Inside each classroom, there are children's restrooms, and each have two (2) toilets and a child size sink available, as well as changing tables. Classrooms A and B include a staff restroom. There are additional staff restrooms located on the school grounds. According to the Director, children are currently offered water from a water jug and disposable cups. The water is from a water vendor (Sparklets).

There is napping equipment in the Head Start programs, children nap and bring napping items from home on Monday and is sent home on Friday to be laundered. Napping cots are located in each classroom and are labeled with each child’s name, sanitized once a week and additionally if needed.

The outdoor play areas are fully fenced. Outdoor play equipment was inspected for safety, free of sharp, loose pointed parts, good repair and age appropriateness. The surface of the outdoor activity area was observed to be maintained in a safe condition, and free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There are required shade with pop-up tents and trees. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards were observed.

Children's records and staff files were reviewed. All staff is fingerprint cleared, and CPR/1st Aid and Mandated Reporting Training reviewed. Staff members provided proof of current Pediatric First Aid and CPR certification. The name of the childcare center director or fully qualified teacher(s) designated to act in the director's absence has been reported to the department.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Director provided LPA an updated Facility Roster un-associate staff who no longer work at the facility.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA verified that the lead testing was completed in accordance with the Written Directives outlined in PIN 21-21.1-CCP.

LPA discussed the safe sleep regulations with licensee [or facility representative] and discussed the Child Care Licensing Safe Sleep web page at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

A notice of site visit was given and must remain posted for 30 days.
