On 03/16/2023 at 9:10 AM an unannounced Required – 1 Year Inspection was conducted by Licensing Program Analysts (LPAs) Katrina Chicote and Franchesca White. Facility is currently licensed for a capacity of 73. Licensing staff announced purpose of visit and initially met with Facility Representatives (FR), who granted entry to facility. Facility is located on Lindbergh Elementary School campus in RMs 37 and 1. The program currently operates a Full Day Program from 7:15 AM to 4:45 PM . LPAs singularly toured the facility both indoors and outdoors. LPAs observed required licensing postings in classrooms and in Main Office.

LPAs observed RM 37 playing outside upon arrival with 23 children and three staff (S1, S2, and S3) and RM 1 using bathrooms and transitioning out of classrooms with 20 children and three staff (S4, S5, and S6). At the initial start of the site visit there is a total census of 43 children and a total census of six staff present during today's inspection.

Beginning at 9:37 AM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPAs observed wooden floors and rugs through out and to be free of hazards at time of inspection. LPAs observed child size tables and chairs available for children well maintained and learning stations for dramatic play, math, art, and science with toys also well maintained free of loose and sharp parts. LPAs observed a library area with soft seating available for quiet play in each classroom. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Mats/cots were inspected. Per FR, linens are provided by the facility and washed weekly by facility. Availability of drinking water was reviewed. LPAs observed water bottles available for children in each classroom. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPAs observed bathrooms available in RM 37 to have one toilet and one sink. LPAs observed additional bathrooms in Main Office with one toilet, one urinal, and one sink and bathroom next to RM 1 to have three toilets and four sinks. Facility has 5.5 toilets and six sinks total available for children.

Snack/lunch menus were reviewed and observed posted at time of inspection. All meals and snacks are provided by the Lynwood Unified School District and delivered to facility. Children are provided with a Breakfast, Lunch, and snack. Children eat lunch in Main Office, LPAs observed child size tables and chairs available for eating. Trash cans used to discard food have tight fitting lids.

LPAs observed first aid kit is kept in each classroom and was inventoried for necessary supplies. Carbon monoxide detectors were tested and observed operable in each classroom. LPAs observed smoke detectors observed present in the facility and fire extinguishers have been serviced on 10/11/2022. LPAs observed disaster drill log available, last drill conducted on 02/15/2023. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Per FRs, C6 requires medication that is kept in school's Nurse's Office, LPAs observed proper documentation in file at time of inspection.

Beginning at 9:49 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPAs observed age appropriate multi-level play structure with slides and four swings that has cushioning material to absorb fall that is all well maintained. LPAs observed bicycles available for children to use outside that are well maintained. LPAs observed large free space play area with cushioning material and grassy area available for children in outdoor play space. Required shade and fencing were inspected. LPAs advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to their water bottles and water fountains during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPAs observed sign in and out sheets and facility also uses electronic sign in/out. Personal Rights of children were observed by LPAs.

Beginning at 11:02 AM, staff and children’s records were reviewed. Children’s Records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857. The review of Staff records was documented on the LIC 859.

Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file. LPAs observed Pediatric First Aid/CPR expires on 03/04/2023 for S4. LPAs did not review confidential staff files during the inspection due to the staff confidential files being stored at the administrative office. All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Lynwood Unified School District.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The following were discussed:
The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed.

Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. LPAs advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 03/16/2023.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
