On 6/28/2024, at 8:45 am, Licensing Program Analyst (LPA) Carolyn Tuba conducted an unannounced Annual inspection. LPA entered facility through the main school district office. The Child Development Supervisor’s office is located at the main entrance. LPA met with Child Development Supervisor, Gabriel Munoz and who guided the LPA on a tour of the facility indoors and outdoors which took approximately 1 hour and 40 minutes. This is a State preschool program licensed for 384 children, which operates Monday through Friday from 7:00 AM to 5:30 PM. The Child Development Supervisor stated there are 251 children currently enrolled. The facility is part of the Hacienda La Puente Unified School District. LPA observed a total census of 184 children with 28 staff members taken. LPA conducted consultation during this inspection.

The following classrooms were inspected: Room 23 3-year-olds with 18 children and 2 staff members, classroom has 1 sink. Room 24 3-year-olds with 12 children and 2 staff members, classroom has 1 sink. Room 25 3-year-olds with 16 children and 3 staff members, classroom has 1 sink. There is a Girls bathroom located right outside of the classrooms that has 3 child size toilets and 2 child size sinks with a changing table and a Boys Bathroom with 1 child size toilet, 3 smaller urinals and 3 child size sinks with a changing table. Room 26 older 2’s and 3-year-olds zero census and 1 sink. Room 27 2-year-olds with 10 children and 3 staff members, classroom has 1 sink. Room 28 there was zero census and is be used for storage only. Room 29 3-year-olds with 15 children and 2 staff members and includes 2 children size toilets with 3 child size sinks, changing table and teacher’s office. Room 30 2-year-olds 7 children and 2 staff members are temporarily using the classroom due to classroom H being repaired. Room 31 3-year-olds with 14 children and 3 staff members, classroom has 2 child size toilets and 2 child size sinks with changing table, teacher office, small kitchenette, additional sink and staff bathroom. There is a small break room between Room 29 and 30 with additional staff restroom. Room H 2-year-olds with zero census and classroom has 2 child size toilets with 2 child size sinks and an additional sink, kitchenette, teacher office, changing table and staff bathroom. Room J 2-year-olds with 5 children and 2 staff members and classroom has 2 child size toilets with 2 child size sinks and an additional sink, kitchenette, teacher office, changing table and staff bathroom.

Room 22 4-year-olds with 17 children and 2 staff members, classroom has 1 sink (Per Child Dev. Supervisor would like to utilize the room for children exploration). Room 21 4-year-olds with 16 children and 2 staff members, classroom has 1 sink. Room 20 4-year-olds with 19 children and 3 staff members, classroom has 1 sink. Room 19 4-year-olds with 18 children and 2 staff members, classroom has 1 sink. Room 18 4-year-olds with 17 children and 2 staff members, classroom has 1 sink. Room 17 4-year-olds with 0 census, classroom has 1 sink. There is a Girls bathroom located right outside of the classrooms that has 3 child size toilets and 2 child size sinks and a Boys bathroom with 1 child size toilet, 4 smaller urinals and 2 child size sinks. Staff members are fingerprint cleared through the school district. Licensed facility is within the conditions, limitations, and capacity specified on the license. The facility has additional staff members to support the facility and assist with preparing meals and distributing to the classrooms, as well as any other staff and children’s needs. LPA inspected the cafeteria where children and parents use to drop-off and pick-up their children. (LPA too photos)

The classroom furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. Indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. Telephone service, heating, lighting, and ventilation were evaluated. Storage for children’s belongings and napping equipment was inspected and meet all regulatory requirements. Children bring their own bedding, which are sent home at the end of each week to be laundered. Facility has spare sheets if children forget to bring from home. LPA observed that some children utilize blankets instead of a sheet and it was advised that crib sheets be used so children do not end up on the cot’s surface. (Technical Advisory was issued).

Availability of drinking water was reviewed and per Child Development Supervisor facility provides water in pitchers with disposal cups and children have the option of bringing water from home. Toys and surfaces are observed to be safe and clean. There are 3 staff restrooms available near the main lobby of the entrance. LPA observed a staff lounge and staff workroom.

Carbon monoxide and Smoke detectors are connected to the Fire Department and are tested annually. Fire extinguishers are serviced annually and were last serviced August 24, 2023. Storage cabinets in classrooms were inspected and no hazardous items were found, cleaning items are kept inaccessible to children in care. No poisonous cleaning compounds were found in the childcare center. Emergency disaster drill was last

conducted on February 28, 2024 at 8:30 am. There is an Emergency Disaster Plan binder that Child Development Supervisor shared with LPA of evacuation plans and designated staff who have specific duties during an emergency situation.

There is a First Aid kit available in each classroom and was inventoried and found to be in good condition with all necessary items.

LPA inspected Cafeteria kitchen where meals are prepared and distributed to each classroom. LPA inspected commercial size refrigerators. There is an allergy list, and the kitchen is inspected by the School District’s Food and Service Supervisor, as well as the Food and Health Department annually. Each classroom has a small refrigerator for storage. Food preparation areas were toured for safety, cleanliness, and proper equipment. Breakfast, Lunch and PM snacks are served by the facility. Menu is posted in each classroom and if changes are made to the menu, facility will update the posted meal menu. LPA observed parent board with all required documentation located in each classroom. The license is posted in the main lobby.

LPA was informed that there are currently children on medication at this time, Child Development Supervisor has an IMS plan and documents when medication is administered with a date, time and signature as well as additional staff witnessing it. LPA reviewed and provided consultation. Director stated that medication would be kept in the main office and there is currently a small fridge for medication that requires refrigeration. LPA advised that Medication should be given back to parents/guardians when child leaves the facility or medication expires.

Outdoor playground equipment is in safe condition, free of sharp, loose, or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. There are currently 6 playgrounds and 4 of the 6 playgrounds have a play structure with foam material to cushion falls. One of the playgrounds has a big wooden trolley with a bike path and bike equipment. The other playground has tables and grass areas for children to play.
All 6 playgrounds have adequate shade with trees or shade covers. The outside school yards have cameras that are connected to the Police Department and Child Development Supervisor has direct access. There are sheds with bikes and outside equipment. The playground is surrounded by a covered chain-link fence that are at least 6 feet tall. (LPA took photos of all playgrounds)

Sign in/out is done using a sheet designated for each student. According to the Child Development Supervisor the parents are required to show their identification each time they pick-up due to volume of children at this facility. There are a few staff members who are designated in the cafeteria where drop-off and pick-up occur, as well as a white board that is updated for each classroom census as children arrive and depart the facility. Office Admin and Staff have radios to communicate with each other and each classroom has a phone with intercom for communication and speakers throughout the facility. (LPA took photos)

LPA reviewed staff records and found that they were complete. Clearance for adults and verification of CPR/First Aid, educational requirements, immunization records, mandated reporting and health preventative practices documentation was reviewed. Children’s records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file.

LPA was provided with Parent Handbook, Meal Menus and Incidental Medical Plan documentation.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. LPA reminded that lead results need to be posted for parents to view until the next testing.

Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

AB1207 Mandated Child Abuse Reporting – On or before March 30, 2018, any person who works in a childcare facility shall complete the training and renew the training every 2 years.
