On December 27, 2022 at 8:00 AM, Licensing Program Analyst (LPA) Monique Ayala conducted an announced inspection at the facility noted above and met with Director, Norma Amezcua. The purpose of the inspection was to conduct a Case Management - Licensee Initiated inspection for a pending application. There is also a pending application for a new infant care center located at the same address.

The facility is currently licensed to care for a total capacity of 64 children, ages 2.5 until entry into kindergarten. The applicant is requesting a Capacity Change to care for a capacity of 40 children, ages 2 until entry into kindergarten. The facility was granted a fire clearance for the requested capacity on September 22, 2022. The facility consists of three (3) classrooms: Room 1, Room 2, and Room 3. The facility also has two (2) outdoor play yards: Preschool Play Yard and Toddler Play Yard. The operating hours for the facility is Monday through Friday from 6:30 AM to 5:30 PM.

All areas identified on the facility sketch were inspected. LPA was guided on a tour of the facility accompanied by the facility representative. Sign-In and Sign-Out is conducted outside each classroom. Per Director, a health check is conducted prior to children being signed in on a daily basis. Upon arrival, LPA observed 3 children and 2 teachers/staff (S1 and S2) in the Toddler Outdoor Play Yard; 1 child and 1 teacher/staff (S3) inside Room 2, and 9 children, 1 teacher/staff (S4) and 1 teacher aide/staff (S5) inside Room 3.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby in each classroom to store their belongings. LPA observed cots inside each classroom. Per Director, sheets are washed at the facility twice a week or when soiled. The facility provides children with meals and snacks. First Aid supplies were observed inside each classroom. Per Director, medication is only administered to a child when accompanied with a doctor's note and medication is store in each classroom.

Per Director, the isolation area is located in the office. In the office, a cot can be placed on the floor for an ill child and the staff restroom is available if an ill child requires it. Age appropriate sinks and toilets were inspected for availability and good repair. General sanitation was observed inside each restroom. Availability of indoor and outdoor drinking water was observed (water jugs and disposable cups).

Smoke detectors and carbon monoxide detectors were tested and are operable. The fire extinguishers are fully charged and were last serviced on October 12, 2022. Per Director, transportation is not provided by the program. The facility has verification of disaster and fire drills. The last drill was conducted on December 15, 2022.

Disinfectants, cleaning solutions, storage for medication and other items that are dangerous to children, were inaccessible to children in all areas. Director stated that there are no poisons stored at the facility. The kitchen/food preparation area and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste have tight-fitting covers that are kept on and in good repair.

The facility has outdoor play equipment that is in a safe condition, free of sharp, lose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Areas around or under high climbing equipment, and similar equipment are cushioned with material that could absorb a fall. LPA observed a large sandbox located in the Preschool Outdoor Play Yard. Per Director, the sandbox is inspected by staff prior to use each day. LPA observed adequate shade in the outdoor play yard. There are no bodies of water on the premises.

All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person present with a valid Pediatric First Aid/CPR certification (EMSA-approved) during this inspection.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed one child's (C1) record, and administrative records. Based on measurements taken during today's inspection, including the availability of 4 toilets and 6 sinks, the facility is equipped to care for the requested decrease in capacity of 40 preschool children.

No deficiencies were cited during today's inspection. A Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days.
