25 | At 9:00 AM Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced required inspection. A risk assessment was conducted prior to entering the facility appropriate PPE was used. Upon arrival LPA observed 18 children and 4 staff sitting on blankets outside. Facility was participating in the California’s Great Shake Out. LPA joined the great shake out until it was all cleared. LPA met with Teacher Elsa Guerrero, who called the main office for support. At 9:45 am Program Facilitator Claudia Casillas arrived and took over the inspection. This is a preschool age head start program licensed for 44 children which operates a two morning session classes Monday – Friday 8:00 am to 11:30 am with 10 children enrolled in each classroom.
All areas identified on the Facility Sketch were inspected. Facility is located in a double bungalow in the Cogswell Elementary School Campus. The following staff was present during this inspection: Staff # 1, Staff # 2 with 10 children and staff # 4, staff #5 with 8 children. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. All children were observed to be under supervision, including visual supervision, of a teacher at all times. The licensed facility is within the conditions, limitations, and capacity specified on the license.
LPA observed the facility to be clean, sanitary and in good repair. Furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. Classroom was inspected to ensure that the floors have a surface that is safe and clean. All toilets and washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, an outdoor blue bench/ table is used for isolation. Per Program Facilitator if child needs to isolate there’s always a teacher with them. Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted.------------------------------------------- pg 1 of 4 -----------------------------------------------
