25 | Licensing Program Analyst (LPA) Nolan Tcheng conducted an unannounced Case Management-Deficiencies inspection. Upon arrival at 11:40am, LPA met with Director Diane Connell, to whom the purpose of the inspection was explained. There were children present during the time of inspection.
Census was taken. There were 38 children with 8 staff members.
During an inspection on 08/16/2023, LPA became aware of cases of Hand, Foot, and Mouth Disease (HFMD) at the facility during the Month of June. LPA reviewed the facility file and observed no incident report submitted to the Department office during that time for communicable diseases. LPA discussed with Director the importance of submitting a report to the Department and to Public Health when there is a outbreak of a communicable disease, such as HFMD, at the facility. Facility did provide screenshots of a message from ProCare App on 06/15/2023, indicating that they followed protocols and informed parents of the outbreak, what steps to take, and when they are able to return.
Families were informed and children were sent home during the outbreak. Health and Safety protocols were followed. LPA is issuing an Advisory Note Technical Violation to address the Reporting Requirements not followed.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
Exit interview was conducted with Director Diane Connell, at 12pm. Copy of report provided.
