Prior to entrance to the facility, LPA Lucero conducted a Covid-19 assessment and based on the facility representative's responses to the facility assessment questions, LPA Lucero determined it was safe to proceed. On Wednesday, November 10, 2021 at 10:00am, Licensing Program Analyst (LPA) Lucero conducted an unannounced Required One Year Inspection. LPA met with Diana Montoro, Head Teacher &
Alicia Rosas, Office Administrator. Facility is currently licensed for a capacity of 125 preschool children. Licensing staff was taken on a guided tour of the facility of both indoors and outdoors by Diana Montoro.


At approximately 10:15am, LPA conducted the initial start of the site visit, there is a total census of 12 preschool children with three staff in Classroom 101, 19 preschool children with three staff in Classroom 102, 12 preschool children with three staff in Classroom 103, and 12 preschool children with 3 staff in Classroom 104.

Furniture and equipment was inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Storage for children's belongings was observed. Mat/cot was inspected. Availability of drinking water was reviewed. LPA observed that children have water bottles available to them and are placed on a wheeled cart. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. Toys observed to be clean.

A first aid kit is kept in the classroom each. Carbon monoxide and smoke detectors is routed/hardwired through the facility building. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. Per facility representative, facility provides a morning and an afternoon snack. Lunch is also provided by the facility.

At approximately 10:45am, outdoor equipment was inspected for safety, cushioning material, good repair and appropriateness. Required shade, drinking water and fencing were inspected. LPA Lucero advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to drinking fountains during outside play, and also have their water bottles wheeled out to them on a cart. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

At approximately 12:30pm, Staff and Children’s Records were reviewed. Inspection of required forms made. CPR/First Aid for Staff #1 was issued by the American Red Cross. LPA observed that the certificate states Adult and Pediatric First Aid/CPR/AED, was completed on 04/10/2021 and is valid for two years.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. During records review, it was determined that Teacher Crystal Palos is fingerprinted; however, she is fingerprinted under the incorrect ORI Number. Teacher Crystal Palos' physical file could not be located at the time of inspection. An interview with Teacher Crystal Palos who stated that she began working at the facility in December 2020. A Technical Violation was issued.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA advised Representative that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check.

No deficiencies were cited on this date. Exit interview, copy of report was given. Appeal rights were issued and discussed.
