On 10/12/22 at 9:45 AM Licensing Program Analysts (LPAs) Austin Estrada and Jeanette Estrada conducted an unannounced Required Annual Inspection. LPAs met with Director Debra Ward, who provided a tour of the facility. Per Director, the hours of operation are Monday - Friday 7:30am to 5PM. The facility has four classrooms where care is provided. Classroom 101 contained 16 children, four Staff and one student worker. Classroom 102 contained 14 children, five teachers, and two student workers. Classroom 103 contained 16 children, two teachers and two student workers. Classroom 104 contained 16 children, three teachers, and two student workers. Teacher child ratios were observed to be in compliance and staff names recorded. The following was observed during the tour of the facility:

All areas on the facility sketch were inspected. All four classrooms inspected for cleanliness and good repair. The floors were clean and lighting was in operable condition. There were age appropriate toys and equipment. Each child brings their own drinking water which is labeled with their names. Each child had their own cubie/storage area. Each classroom had two bathrooms with one toilet and one sink in each bathroom. All restrooms were observed to be clean and in operable condition. Smoke and carbon monoxide detectors were observed to be in operable condition. LPAs observed Fire extinguisher indicated fully charged with a service tag dated 1/7/22.

The facility provides breakfast, lunch, and afternoon snack to children in care. Menus are posted and are provided to parents a week in advance. Food is prepared in the kitchen area. LPAs inspected the food preparation area and refrigerator and observed it to be clean and in operable condition. All food was observed to be in good quality.

LPAs toured the outdoor play area. Outdoor area was observed to have age-appropriate toys and material for children, free of loose, sharp, and/or pointed parts. LPAs observed required cushioning under climbing structure to absorb fall. Shade was observed in the outdoor area. There are no bodies of water on the premises. First Aid kits were inventoried.

Napping equipment and bedding were inspected and found to be clean and in operable condition. Per Director, bedding sheets are washed weekly at the facility. The isolation area is located in the main office.
The facility administers medication, LPAs observed medication to be locked in a cabinet in the office inaccessible to children in care. Medication is administered on a as needed basis.

In classroom 101, cleaning supplies were observed to potentially be accessible to children. LPAs advised the Director to move the cleaning products to an inaccessible area. Director moved cleaning supplies to an inaccessible area.

Children’s records were reviewed and were complete. Staff records were reviewed. Two out of five staff records reviewed did not have the required Mandated Reporter Training. One Type B violation is being issued due to Mandated Reporter Training not being completed by two staff members.


The deficiencies listed on the following page (LIC 809-D) were observed by the LPAs and are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.



Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPAs informed licensee of the importance of checking for recalled devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
