13 | Licensing Program Analyst (LPA) Cynthia Reyes, conducted an unannounced follow up inspection to present the findings of the complaint investigation. LPA met with Tom McFadden, Director. A Covid-19 risk assessment was conducted prior to entering the facility. No children in care due to the day care is closed for the summer until August 19th.
During the course of this investigation LPA Reyes conducted interviews with staff, children, reporting party (RP) and other witnesses. It was stated that a day care child was choked while in care and the day care child was also called an inappropriate name. Per several interviews there were no witnesses to a child being choked, although it was stated other staff and children were present when the alleged choking and inappropriate name was called. Per the police report and medical report obtained, no crime was committed and no signs of any injury. There was no evidence showing the child was choked. LPA was unable to interview the victim per the complainant and was unable to interview another witness as they would not respond or return the LPAs telephone call.
