Licensing Program Analyst (LPA) Rita Ramos conducted an unannounced case management inspection on 03/12/2020 to the above facility. LPA arrived to the facility at 8:05AM. Upon arrival, LPA met and greeted Dane Joseph, Licensee's nephew who directed LPA to Licensee, Nerris Isaac. Licensee guided analyst on a tour of the facility. There were children no children present upon arrival. Licensee's husband Melvin Isaac and Licensee's daughter Letoya Martin were also present during the inspection.

The purpose of this inspection was to review the Caregiver Background Check Bureau letter for the Confirmation of Removal for Lyra Ramdeen dated March 03, 2020, and to ensure that the individual named is no longer residing, working and/or volunteering at the facility. The purpose of the inspection was discussed with Licensee, Nerris Isaac, and a copy of the Confirmation of Removal letter was provided to the Licensee during the inspection.

LPA obtained a signed Confirmation of Removal for Lyra Ramdeen during today’s visit.

Based on the evidence obtained during today’s visit, LPA Ramos has verified that the individual Lyra Ramdeen is not present, employed, or volunteering at the facility. Per Licensee, Lyra Ramdeen is no longer residing in the home, however, does plan to request an exemption for Lyra Ramdeen.

Licensee stated at 8:21AM that they do not have any children currently enrolled at their facility. LPA, however, provided the Licensee with the Family Child Care Home Addendum to Notification of Parent's Rights (Regarding Removal/Exclusion) LIC 995B form during the inspection. LPA explained to the Licensee that if any children were to be enrolled, the form is to be given to the parent's and signed.

At 8:41AM, LPA asked Dane Joseph how long they have resided in the home and Dane Joseph stated, "A couple of weeks." Licensee stated that they will get Dane Joseph fingerprinted and associated to the facility.

Based on LPA's observations, interviews conducted, and records review, Dane Joseph is not listed on the Licensing Information System of individuals who are associated to the facility and does not have a criminal record clearance. A civil penalty of $500 is being assessed during today's visit.

The following deficiency listed on the attached deficiencies page is being cited in accordance with California Code of Regulations Title 22.

A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Licensee/Director was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit. A copy of the Parent Notification Requirements was also provided to the licensee/Director.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Nerris Isaac, Licensee, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
