Licensing Program Analyst (LPA) Thelma Razo called the facility to deliver complaint investigation findings due to COVID-19 and precautionary measures. LPA Razo spoke with Director Pam Deem and discussed the purpose of the call.

On 9/22/2020, LPA Razo made an initial complaint tele-inspection via Zoom and met with Director Pam Deem. At 11:17AM, LPA had a virtual tour of the facility to include classroom A, C, D, restrooms and outside play area. LPA interviewed Administrator during this tele-inspection. Additional interviews were conducted by LPA Razo with 4 parents (via phone at their residence on 11/4/2020, 11/16/2020 and 12/22/2020), Child #1 and Child #2 (via virtual call at their residence on 11/13/2020), and 7 staff (via phone on 11/16/2020, 12/22/2020, 12/29/2020 and 12/30/2020). LPA obtained staff and children's roster, Incident Reports for the month of August and September 2020, Unusual Incident/Injury Report (LIC624) date of incident 7/1/2020 for Child #3 and reviewed information pertaining to Nursemaid's Elbow. On 9/22/2020, LPA conducted a tele-inspection with Director Pam Deem and toured three classrooms, A (2 year old), C (3 year old) and D (4 year old). Per Director Deem, all 10 children in room A wear diapers, 2 children in room C wear diapers, and all children in room D are fully potty trained. electronic system. Various interviews have indicated that the facility provides and uses their own diaper brand which is different from the ones being used by the children at home. During interviews conducted, there was a statement from Parent # 2 (P2) who indicated that on one occasion, her child was left with the same wet diaper throughout the day. According to P2, her child, Child #5 (C5) was wearing the same diaper when dropped off. When Staff #1 (S1) was questioned about the reported incident, S1 admitted to have missed a scheduled diaper change which usually takes place after children eat lunch. Per S1, on that particular day, C5 ate lunch and took a nap right after, therefore missing the scheduled diaper change. Per S1, this was an isolated incident that happened with C5 and not C2 who is the subject of the investigation. LPA was unable to confirm records of diaper changes as no specific dates were provided. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation did or did not occur, therefore at this time the above allegation is UNSUBSTANTIATED.

No deficiencies were cited under California Code of Regulations Title 22, Division 12 at this time.
Exit interview was conducted with Director Deem. This report along with a copy of the Appeal Rights will be emailed to Director Deem with a read receipt or confirmation of receipt of email, which will act as the Director’s signature. A copy of the signed report will also be sent by Director to the Department.

A Notice of Site Visit was not provided since a physical inspection was not conducted.
