An announced Case management-Licensee-initiated inspection was conducted on this date by Licensing Program Analyst (LPA) B. Emiko Bell. The inspection was announced because school is currently not in session and as there are no staff or children present LPA had to be let into the facility. The purpose of the inspection was to take measurements of the indoors of room P1, as Pomona Unified School District is requesting a capacity increase. The application was received by the Department on 07/19/19. The Center will open on August 12.

Upon arrival, LPA was greeted by Licensing & Facilities Planner Kym Allen in the parking lot. Mr. Allen then let LPA into room P1.

The classroom is located on the south side of Barfield Elementary School, behind the pre-school playground. Previously, classroom P1 was used as a "social room" for Head Start. It is contained within its own portable, though this portable and the portable which contains classrooms P2 and P3 are separated from the elementary school by a chain-link fence which completely surrounds the playground and both portables. The portable which contains P2 and P3 and the newly-remodeled playground were previously measured on 08/02/18 by the same LPA, and thus, were not measured during today's inspection.

air-conditioning. Overhead lighting provides the majority of the light, but natural lighting comes through the windows as well.

There is no napping equipment in room P1.

The outdoor activity area is completely surrounded by a six foot high chain-link fence. There is foam rubber cushioning underneath the play structure, which has a sign which states it is appropriate for children ages 2-5. There is a HIP structure over the play equipment which provides shade. There is also artificial turf and a tree on the playground. There is age-appropriate play equipment which will be stored in the storage shed which is on the playground.

Each classroom will be on the playground at separate times, so each classroom will be outside for 30 minutes in the morning and 30 minutes in the afternoon. The A.M. session of P1 will be outside from 9:15 to 9:45 and the P.M. session will be outside from 1:00-1:30. There is a water fountain with two faucet heads attached to P1.

There is a fire extinguisher mounted on the wall next to the exit which leads to the playground. The fire extinguisher is classification 2-A:10-B:C and was last serviced 07/02/19. There is a fire alarm system. There is a carbon monoxide detector mounted on the wall next to the child-size sinks; it was tested and is operable.

Mr. Allen assisted LPA with measuring. The encumbered space in the classroom is the built-in storage cabinet with the dry erase board, the children's cubbies, and the kitchenette. The bathroom and the office were not included in the measurements.

Based on measurements taken on this date, the maximum indoor capacity shall be for (28 plus 48) for a total of 76 children. The maximum outdoor capacity shall be for 61 children. Applicant is currently applying for an increased capacity of 20 children.

Fire clearance was granted on 08/06/19 for a capacity of 60 children.

Based upon today's inspection, the classroom appears to meet Title 22 requirements and appears ready for licensure.

The Notice of Site Visit has been posted on the Parent Board. LPA informed Licensing & Facilities Planner Kym Allen. that if the Notice of Site Visit is removed prior to thirty (30) days, a $100 civil penalty may be assessed.

An exit interview has been conducted with and a copy of this report has been signed by and provided to Licensing & Facilities Planner Kym Allen. Appeal Rights have been provided and explained to same.

from the area which contains the portables. The fence is kept locked during operating
hours and are only unlocked when children are being dropped off and picked up. The children will enter through the fence, then step through the gate on to the playground then go through another gate to enter the area where P-1 through P-3 are located. The gate through which to enter the playground and the gate which leads to the portables will also be kept locked during operating hours. In order to gain unannounced entry, an LPA would need to either enter through the elementary school office and walk back to the portables or phone the office. The office would then phone the portables so that staff could let LPA in the chain-link gate, as each classroom has a phone. The phone number for the school is (909) 397-4575.

The Center will operate on the same schedule as Pomona Unified School District. The hours are approximately concurrent as the elementary school, Monday-Friday 7:30 A.M. to 3:30 P.M. The A.M. session will be State Preschool and will run from 7:00-10:30 and the P.M. session will be a Head Start and will run from 11:45 A.M.-3:15 P.M. (This is subject to change per school year per community needs.) The Center proposes to serve children ages 2 until entry into Kindergarten.

The classroom has one door through which the children will enter and one door which leads to the playground. The classroom has two child-size sinks, of which one has a drinking faucet; the sinks are is located directly outside of the children's restroom. The bathroom contains two child-size toilets.

to the left of the entryway. The Emergency Disaster Plan is posted above the phone,
which is located next to the refrigerator. The monthly menu is posted on the refrigerator. All food will be provided by the School District. A.M. snack will be served at 10:15 A.M., lunch will be served at 11:45 A.M. and P.M. snack will be served at 3:00 P.M.

The kitchenette area contains a full-size refrigerator and a microwave, along with an adult-size spigot and a drinking faucet. Medications which require refrigeration will be
stored in a lock box in the refrigerator. Medications which do not require refrigeration will be stored in a cabinet which is designated for the First Aid Kit (there is a label indicating the cabinet) and which is rendered inaccessible by a childproof latch. The cleaning supplies will be kept in one of the cabinets as well, all of which are rendered inaccessible by childproof latches.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. When children become ill during school hours, the Center will call their parent. Though there is no isolation room, ill children may be kept in the office area until the parent arrives to pick them up.



There is age-appropriate furniture and equipment in each room. There is a landline; the phone is located next to the refrigerator. The classroom has central heating and
