Licensing Program Analyst (LPA) Ana Chico, conducted an annual random inspection. LPA met with Leticia Carillo , AM Teacher. LPA observed 14 children present during today's inspection. The Head Start Program operates on the campus of Northam Elementary School. This facility operates an AM and PM session. The schedule is as follows: 8:15 AM - 11:45 AM and 12:15 PM - 3:45 PM. In addition to the Head Start program, Options for Learning also operates a State Preschool Program and a Surround Care School Age on site.

Children attending the preschool program use a portable adjacent to the elementary school. Furniture and equipment were inspected for age appropriateness and good repair. The room is well ventilated via central air/ heat. The refrigerator, microwave and electric oven were observed to be clean. Carpeting and flooring were also observed to be clean and well maintained. A staff and children's restrooms are available inside the class room. Availability of drinking water is provided via water fountain. Cleaning solutions are stored inside a cabinet and/or cabinets that are not accessible to children. Children have cubbies to store their personal items in. A variety of games and other learning materials are available.

The playground is completely fenced with a five foot chain link fence. Children have access to a grassy area, sandbox and an age appropriate playground structure. The playground is well maintained and equipment was observed to be anchored with rubber cushioning underneath to absorb falls. There are shade structures in place and playground toys for children to play with. A water jug with cups are taken outdoors as a drinking source. First Aid supplies are kept in a backpack and were inventoried for quantity and expiration dates. Staff's First Aid is current (ex 11/2020)

According to staff there are currently 32 children enrolled between both sessions. Two Lead Teachers and an Associate Teacher are assigned to ensure that teacher-child ratios are in compliance at all times. Roster was available and up to date. An Emergency Disaster Drill log is kept. The program conducts routine emergency drills (last recorded 4/2019). Per staff, a wellness check is conducted upon arrival. Sign in and out sheets and procedures were reviewed. All children physically present were observed to be signed in.

Menus were reviewed for availability, quantity and appropriateness to children in care. The program offers breakfast and lunch for the AM session and lunch and snack for the PM session. Meals/snacks are provided by Options Food Program which follow mandated Federal Food Guidelines. Meal menus, license, car seat law, parents' rights for centers, emergency disaster plan and all other posting requirements were observed.

This facility provides Incidental Medical Services – IMS . An IMS plan has been submitted.

Criminal Record Clearances are a contingency for hire. All clearances are associated to the New Temple Administrative File #191593520. Mandated Reporter Training Certificate and new immunization requirements are also a requirement for hire. More information can be found at: http:/www.mandatedreporterca.com/training /training.htm. All licensed providers, applicants, directors and employees are required to complete training as specified on the mandated reporter duties and to renew their training every two years. Volunteers are encouraged but not required to take the training. Please review all elements outlined in AB 1207- Mandated Reporting Training.

Per waiver agreement on file for Options, employee and children's records will be reviewed during a collateral inspection at the City of Industry, CA headquarters.


NO DEFICIENCIES ARE BEING CITED IN ACCORDANCE TO TITLE 22 CALIFORNIA CODE OF REGULATIONS.

Upon receipt, the Licensee shall post the Notice of Site Visit. The Notice of Site Visit shall be posted for thirty (30) consecutive days. Failure to maintain posted as required will result in a $100 civil penalty.

An exit interview was conducted and a copy of this report has been signed by and provided. Appeal rights explained.
