Licensing Program Analyst (LPA) Alicia Mooberry conducted a required 1 year inspection to the above facility on 11/30/22. LPA arrived at the facility at 10:00am and met with Marisa Nuno, Lead Teacher and explained the purpose of the visit. Analyst toured the facility. Jackie Ellingson, Education Specialist arrived at the facility during this inspection, the LIC 125 Entrance Checklist was provided to help facilitate the inspection. This is a Head Start preschool program which is operated by Montebello Unified School District and is located on the premises of Bell Gardens Elementary School. The program consists of a full day program in Room 57 and a part-time day program in Room 56. The full day program operates from 7:50am to 1:50pm; the AM Session operates from 7:50am-11:20am, and the PM Session operates from 12:00pm to 3:30pm, Monday through Friday.

Facility is currently licensed for a capacity of 40 ambulatory children and 2 non-ambulatory children for a total of 42 children.

The indoors and outdoors of the facility were inspected. LPA observed 10 children with Staff #1 and Staff #2 on the playground. There were 16 children with staff #3, #4, #5 in Room 56. Teacher child ratios were observed and staff names recorded. The facility is using the ChildPlus electronic system to have parents sign children in and out.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Linens use on cots are washed weekly by the facility, the blankets are washed by parents on a weekly basis. Napping equipment (cots) were observed labeled with children’s names.

A carbon monoxide detector was observed and tested to ensure that it is operable. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed.

Availability of indoor drinking water was observed in classrooms via a water dispenser and disposable cups. Drinking water dispenser and disposable cups are available to children in the outdoor playground.


Per Educational Specialsit, there are no poisons stored at the facility. There are no firearms stored on the premises.

The facility receives all food and meals from the adjoining elementary school. Children receive breakfast, lunch, supper and snack. The facility keeps medications in locked box inaccessible to children. Mediaction observed to be lables and up to date. Refrigerator is available for medication and other items. All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept outside and in good repair.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed via water fountains. LPA advised that no children shall be left without the supervision of a teacher at any time. There are no pools or bodies of water at the facility.

All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free

All staff and employees have obtained a criminal record clearance or criminal record exemption through the Montebello Unified School District. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

Children records were reviewed and documented on the LIC 857. Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed on the sign in APP. Children present were signed in. Lead teacher was advised that a sign in sheet shall be kept for one month and shall be available at the center for review by the Department. ---Page 2 of 3

LPA also reviewed staff records including proof of Pediatric First Aid/CPR and Mandated Reporter. LPA did not review staff records and confidential or medical information during the inspection due to the staff files being stored at the District Office. LPA will review files at a later date and will review for compliance.

Disaster drill log was available, last drill was conducted on 10/20/22.
Facility Representative was reminded to post Menus at lease one week in advance in area visible to children's authorized representatives.



Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.
