25 | Licensing Program Analysts (LPAs) Ashley Calderon and Alicia Mooberry conducted a case management inspection visit regarding incident that was reported to CCLD on 3/26/24, involving two children (Child #1 and Child #2) who are related, incident occurred per parent of children between in 2008-2011. LPA's upon arrival met with Teacher/ Katy Stine and LPA's discussed the reason for today's visit.
During today's visit, LPA's conducted facility tour, collected roster and appropriate documentation's and conducted interviews. LPA's observed 50 children and 4 teachers during nap/ rest time and facility were in ratio. During today's visit LPA's along side with Teacher Katy conducted a tour of the inside and the outside of the facility premises. There are three physical rooms, rooms #3, #4 and #5 and a total of five classrooms. During outside inspection Teacher Katy stated there are two outside restrooms, and 6 outside play areas. Outside play areas include: field, green area, brown area, blue area, water area and bike area and each classroom have their assigned outside area schedule and are not crossing .During visit, Administrator Renee Estrada arrived around 1:45pm informed LPA's she was not present during the time frame Child #1 and Child #2 were enrolled at the above facility, LPA's conducted an interview and Administrator informed LPA's reporting incident to Social Services whom provided a consultation number to provide to parent of Child #1 and #2 if they were to contact center. Staff #1 and Staff #3 were present during enrollment of Child #1 and Child #2, LPA's interviewed staff. LPA Mooberry reviewed three (3) teacher files and LPA Calderon obtained any documentation available for Child #2 and #3, collected roster and recess schedule.
LPA's did not observe any health and safety risks and there were no deficiencies cited. Based on interviews and roster, facility has a plan of supervision for outdoor play, bathroom use and classroom blind spots and per interviews children are supervised during outdoor play, classroom time and bathroom time.
Exit interview conducted with Administrator Renee Estrada and a notice of site visit was given and must remain posted for 30 days. |
