An unannounced annual/required inspection was conducted on this date by Licensing Program Analysts (LPAs) Ariel Cazares and Roxana Lopez. LPAs met with Director Gwen Blackmon who guided analysts on a tour of the facility. There were a total of 17 children and 7 staff.

The infant program is located on the same site as a the preschool component #191501974. The facility’s hours of operation are Monday through Friday, 6:30am-6:30pm. The infant program consists of 6 classrooms: Infant #1-2, Toddler Blue, Toddler Purple, and Green room. There are 2 playgrounds designated for the infant program.

LPAs inspected the classrooms. Each classroom is equipped with a changing table nearby a sink. Children's have personal storage space for their belongings, which are properly labeled. A menu is posted in classrooms for children eating solid foods. Water is available in form of water jugs and sippy cups or bottles. Infant formula is available and Infant Needs and Service plans were reviewed on file. Counter tops were clean and free of clutter or standing food. Cleaning compounds are inaccessible to children.

Outdoor equipment was inspected for safety, cushioning material, good repair and appropriateness. Play area was inspected for hazards and inaccessibility to bodies of water. Drinking water is brought out in pitchers and children's personal cups are used. There is an adequate shaded and rest area.

The facility has a kitchen where snacks are stored and prepared for the children. The hospital caters the children's lunches. The kitchen was inspected and observed to be clean, orderly, and sufficient storage for supplies.

Teacher child ratios were observed to be in compliance and staff names were recorded. Care and supervision was evaluated to determine if the basic needs of infants are met and are appropriate.

Staff and Children’s Records were reviewed for completeness including but not limited to Criminal Records Clearance for adults, qualifications and verification of CPR/First Aid. Staff files did not have proof of immunizations and health screening. Sign in and out sheets were reviewed. Upon review of class rosters, LPA observed that the Toddler Purple room has two children enrolled who are the age of 2. These children are determined to be of preschool-age.

LPA reviewed SIDs, Never Shake A Baby, and safe sleeping practices. Infants should sleep mouth up, on their backs, free of clutter surrounding their sleeping space. A copy of Safe Sleep Concepts were provided at this inspection.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. See attached deficiencies page for citations in accordance with Title 22 California Code of Regulations. Exit interview conducted with Director Gwen Blackmon. A copy of this report and appeal rights were provided and explained. Notice of Site Visit must be posted for (30) days. Failure to do so may result in a $100.00 civil penalty.
